LEASE AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Lease Agreement (hereafter the “agreement”) made and entered into this ____ day of
_________________ by and between:

---------------- of legal age, Filipino, and with residence and postal address at Brgy.
Akle, San Ildefonso, Bulacan, herein referred to as the LESSOR;

and

_______________________ a corporation duly organized and existing under the laws


of the Philippines, with site office located at Km. 96, Mc.Arthur Highway, Brgy.
Tabun, Mabalacat, Pampanga hereby represented by its President, ARNOLD A.
DIZON, herein referred to as the LESSEE;

(Collectively, the “Parties”)

WITNESSETH: That -

WHEREAS, the LESSOR is the owner of a residential house located at


Sitio Narra, Brgy. Akle, San Ildefonso Bulacan (the “property” or “premises”);

WHEREAS, the LESSEE intends to lease the subject property for a period of five (5) months
to commence on August 10, 2017;

NOW THEREFORE, for and in consideration of the foregoing premises, the parties hereby
agree to the lease of the subject property, subject to the following terms and conditions:

1. The LESSEE agrees to pay a monthly rent of ______________________, inclusive of the


value-added-tax.

2. In addition, the LESSEE agrees to pay the amount of Ten Thousand Pesos (PhP10,000.00)
representing one (1) month advance rent and Ten Thousand Pesos (PhP10,000.00) as one (1)
month deposit upon signing of this agreement.

3. The LESSEE shall pay the bills for electricity and water utilities during the term of this
agreement.

4. The term of this lease is five (5) months from ______________________with option to
renew the same as provided hereunder.

5. The property will be used mainly for residential purposes. Any business activity inside the
premises should have the consent of the LESSOR.

6. Any illegal activity made by the LESSEE inside the premises will automatically terminate this
agreement and all advances and deposit will be forfeited in favor of the LESSEE.
7. Only the employees and/or agents of the LESSOR are allowed to stay in the premises, and the
LESSOR reserves the right to terminate this agreement if the number of occupants exceeds
twenty (20) without the consent of the LESSOR.

8. Cleanliness of the premises should always be maintained and garbage properly disposed and
managed; pets will only be allowed upon approval of the LESSOR; and, no funeral service
will be allowed inside the premises.

9. The LESSOR shall be free from liability in case of accident and any damage or loss due to a
natural calamity suffered by the LESSOR or any third party inside the premises during the term
of this agreement, unless the same can be attributed in whole or in part to an act or omission,
or negligence, of the LESSOR. In this regard, the LESSOR warrants that the premises and
all its fixtures and effects are in good and safe condition.

10. The LESSEE is not allowed to transfer or sub-lease the premises or any part thereof to any
third party without the consent of the LESSOR; and the LESSOR has the right to inspect the
premises with due notice to the LESSEE at any reasonable hour to ensure compliance with this
provision.

11. The LESSEE is not allowed to construct, modify or remove permanent fixtures in the premises
without the written consent of the LESSOR.

12. After the initial term of this agreement, the lease may be terminated or renewed every month
thereafter. Any renewal of the lease will be under the same terms and conditions unless a new
agreement is signed by the parties. However, the LESSOR reserves the right not to renew the
lease in case he or any of his family will use the premises for residential purposes by providing
written notice to the LESSEE at least one (1) month prior to the termination or non-renewal
of the lease.

13. The LESSEE has the option to pre-terminate the lease, provided the LESSOR is notified one
(1) month in advance.

14. This agreement may be terminated for non-payment of rent for two (2) consecutive months,
and for violation of any of its provision.

15. Upon termination of the lease, the LESSEE shall turn-over to the LESSOR the leased
premises, including all furniture, fixtures and effects, without any damage, ordinary wear and
tear expected. Any such damage found to have been caused by the LESSEE and/or repair as
may be necessary, will be charged to the advance payment of the LESSEE.

16. The parties shall achieve to resolve any issue or dispute in relation to this agreement or any
provision thereof between themselves. Any dispute not resolved may be submitted for
arbitration in accordance with law.

17. The parties to this agreement undertake to faithfully and fully comply with the covenants
contained herein
