This is to certify that Mr. Jonathan B. Paglingayen, RN, MANc, a 25 year
old, Male/ Single from #122 San Nicolas Norte, Agoo, La Union, Philippines has been
employed as a Staff Nurse - March 01, 2015 to July 15, 2017 and as a Nurse
Supervisor since July 16, 107 up to present.
In Addition, it certifies that Nurse Jonathan had experienced working in the
Emergency Room, Out-Patient Department (OPD), Psychiatric Ward, Medical Ward
(Male/Female) and Pediatric Ward. He performed the following duties and
responsibilities as a Nurse, to wit:
ᵜ Provided general nursing care to patients within the assigned area/unit;
ᵜ Carried out admission and discharge orders accordingly;
ᵜ Monitored patient’s conditions like vital signs, intake and output monitoring, etc.;
ᵜ Assisted Doctors during rounds and/or physical examination;
ᵜ Administered intravenous, intramuscular, oral medications as ordered by the physician;
ᵜ Completed records of nursing observations and patient’s charts;
ᵜ Conducted health teachings to patient considering the specific illness;
ᵜ Observed Nursing Law and Ethics (R.A. 9173).
