JUDICIAL AFFIDAVIT

I, Accused Name, Filipino, of legal age and with postal address

at 123 Guadalupe Bliss Barangay Cembo, Makatyi Cit, after having
been duly sworn in accordance with law, hereby depose and state by
way of answer to the question propounds by the undersigned counsel
at CEU Legal Clinic, Sen. Gil Puyat Ave.,Makati City.

I further state that answering the questions asked of me, I am

fully conscious and do so under oath, and that I may face criminal
liability for False Testimony or Perjury.

Offer: The testimony of the Accused is hereby being offered to

prove that he is innocent of the crime charged, to prove the
allegations contained in the Information for Grave Treats which was
docketed as Criminal Case No. 123 and to identify certain documents
for the defense.

Purposes

Lawyer: The testimony of the Accused _ is being offered for

the following purposes;

i. To prove that none of the elements of the crime of Grave

Treat exists in this case and that the accused is not guilty of
the crime charged for she did not commit the alleged offense
against the Private Complainant;

ii. To prove the fact that the Private Complainant was not in the
 performance of her duty during the incident, therefore one of
the elements of the crime charged is missing;

iii. To prove the fact that the Private Complainant was not
attacked by the accused;

iv. To prove the material allegations in the Counter-Affidavit of

the accused in this case;
