AFFIDAVIT OF CONSENT

I, BONIFACIO ARAGONES, of legal age, Filipino, married and a resident of

Candau-ay, Dumaguete City, Philippines, having been duly sworn in accordance
with law, hereby depose and state that:

1. I am one of the co-owners of lot situated in the barangay of Candau-ay,

Dumaguete City, Negros Oriental, known as Lot 6065, covered by Tax
Declaration No. 14-0019-00214;

2. The registered owner of the above described property, who is Juana Pino, is
my deceased mother. Thus, I have acquired rights and interest of the
property by virtue of succession;

3. I hereby signify my written consent, freely, willingly and voluntarily to allow

DHALIA ARAGONES to build her residential house on a portion of the said
parcel of land and consequently, have a water connection thereon;

4. I will take all responsibilities and obligations arising from the grant of this
consent to DHALIA ARAGONES. Hence, I undertake to hold free and
harmless the officers and employees of the City Government of Dumaguete
and Dumaguete City Water District (DCWD) from any adverse claim or any
conflict arising between the owner(s) and/or heir(s) of the property involving
herein service connection;

5. I am executing this affidavit to attest the truth of the foregoing facts as to my

undertaking to give consent to DHALIA ARAGONES to support her
application for Water Connection Permit.
