Authorized, Affected and Other employees must all receive some form of LOTO training in a
facility).
2. T The 4 essential elements in a Lockout/Tagout program are procedures, training, devices and
periodic inspections.
3. T Always disconnect the energy source after turning the machine off. (Note: If you disconnect a
machine under load, you risk the possibility of arcing or explosion.)
4. F Using the ON/OFF switch on a machine is a good way to isolate the energy sources going to that
machine. (Note: Action must be taken at the electrical disconnects, breakers, valves, etc…
Using the ON/OFF switch does not isolate the energy coming into the machine).
