COOPERATIVE TRAINING AGREEMENT

1. The purpose of this agreement is to :1) define clearly the conditions and the
schedule of training whereby the ___________________________ student is to
receive training as an OJT trainee, 2) serve as guide to the cooperative parties
and assist the Camarines Norte State College – College of Engineering in
providing the student with the opportunities for training in essential skills of the
occupation and the technical information related to it.

2. In order that the systematic plan which provides for well rounded training can be
followed, a schedule of work experience on the job and technical information
paralleling it to what is taught in school have been worked out and agreed upon
by the cooperating agency and the representative of the school.

3. The course of instruction is designed to run for 300 hours with a minimum of 40
hours per week required for the work experience.

4. The student agrees to perform diligently the worked experience assign to him/her
by the cooperating agency. The student also agrees to pursue faithfully the
prescribed course of the study and to take advantage of every opportunity to
improve his/her efficiency, knowledge and personal traits so that he/she may
enter his/her chosen occupation as a desirable employee at the end of the
training period.

5. The student, while in the process of training, will have the status of student
learner neither displacing a regular worker now employed nor substituting for a
worker needed by the cooperating agency.

6. The training period begins on ___________________, 20____ and ends on

7. All complaints regarding the conduct and performance of the student-learner

shall be made to the school through the OJT coordinator.

8. The OJT coordinator shall have the authority to transfer or withdraw the student-
learner at anytime.

9. The On-The-Job-Training (OJT) of the student-learner shall conform to all rules

and regulations. The school and the cooperating agency shall not be held liable if
the student-learner meets an accident outside the school and in and out of the
agency compound.

COLLEGE OF ENGINEERING
TRAINING, AGREEMENT AND LIABILITY WAIVER

I, _________________________ a student of Camarines Norte State College,

hereby voluntarily agree to undergo on-the-job-training; which is an academic
requirement for the graduation of the Camarines Norte State College – College of
Engineering and Industrial Technology, under its BS _______ Engineering Curriculum
under the following terms and conditions.

1. That shall abide by all the company’s / agency’s rules and regulations and shall
comply with those imposed for the training otherwise I shall be excluded form
further participation;

2. That there is no labor- management relationship between me and the

corporation;

3. That I shall exercise care and diligence in any task assigned to me;

4. That I shall renounced and waive any claim

against______________________________ and the Camarines Norte State
College – College of Engineering for any loss that I suffer personal of pecuniary
in the performance of my duties and functions while under training; and

5. That I shall be made answerable for any and all liabilities for damage to property
of injury to the third persons, which may be occasioned, by my intentional or
negligent acts while in the course of training.
