LAST WILL AND TESTAMENT

KNOW ALL MEN BY THESE PRESENTS

I, _______________________, of legal age, single, a citizen of the Philippines and currently

a resident of Brgy. Banate, Malungon, Sarangani, with sound and disposing mind and
memory, and without having been forced, intimidated or unduly influenced by anybody,
have hereby voluntarily executed and proclaimed this instrument, as my Last Will and
Testament, in English, a language I speak and write with and of which I am well conversant:

1. That should I finally rest in eternal peace, it is my wish and desire that internment, vigil
and burial be made in accordance with the customs and traditions of the Catholic Church;

2. That I am currently employed as a Seafarer of NYK FIL-SHIP MANAGEMENT

CORPORATION, with principal address Sta. Pontenciana, Intramuros, Manila;

3. That being an employed Seafarer, I am granted benefits by the company during

my employment and even after employment in the form of death benefits, disability benefits,
and all other benefits provided for by the corporation in compliance with existing laws;

3. That should the Lord Almighty finally summon this soul from its earthly abode, it is my
wish and desire to bequeath, grant and devise my properties above-mentioned, as follows:

A) To my beloved child ___________________, I hereby bequeath all my properties and

benefits in relation to my employment as Seafarer;

4. However, considering that ____________________ is still immature and cannot

manage money properly, I hereby appoint my mother _____________________ to be the legal
guardians and trustees of all my properties and benefits to be claimed for the benefit of the
beneficiary/heir ________________________ until she fully matures and be responsible
individual;

5. That the usufructuary rights over all my properties shall pertain to the trustees during
their lifetime and only provide ________________________ the necessary support, monthly or as
she needs from the benefits abovementioned and only upon their demise shall the provisions
of Paragraph 2, Sub-Paragraph A come into effect;

6. That for the purpose of rendering this Last Will and Testament effective thru the
proper proceeding in Court, I hereby name and constitute __________________ as Executor
and Administrator of this Last Will and Testament, and that in his incapacity, I hereby name
_______________________ as his substitute;

7.That the Executor and Administrator I hereby nominate shall be excused from posting
any bond;

8. That I hereby revoke, set aside, and annul any other will or testamentary disposition
I have made, signed, or proclaimed.

acknowledged to me that the same are their free acts and voluntary deed.
