AFFIDAVIT
I, KAREN TIGA ESTRADA, of legal age, single/married, Filipino and with residence at
Zone II, Cawit, Zamboanga City, Philippines, after being duly sworn to oath in accordance with
law, hereby depose and state :

That I am the driver of that the following described motor vehicle registered in the name
of my _______________, NATHANIEL E. TIGA, to wit :

MITSUBISHI, with Engine Number-3A92UCD3536, Chassis Number-

MMBSNA13AFH007563, and Plate No. ABU5592, covered by Certificate of
Registration No. CR-269488800, dated 08/31/2018 and Official Receipt
No.1553759690, dated 04/11/19, both of the Land Transportation Office,
Zamboanga City District Office;

That on April 14, 2019 on or about 04:15 in the afternoon, while maneuvering the said
motor vehicle to park at our residence at Cawit, City of Zamboanga;

That upon return to my parked vehicle, I was surprised that the front bumper of my motor
vehicle was damaged, and I assumed that the motor vehicle accidentally hit the gate and sustain
damages, to wit :

1. Front bumper – scratches ;

2. Rear bumper– scratches

WHEREAS, the rear bumper of said motor vehicle was painted using Pylox Paint.

That I am executing this affidavit to attest to the truth of the foregoing narration of facts
and for whatever purpose this may serve.

I, MARISSA FATIMA RUSTE FABIAN, widow, of legal age, Filipino and a resident of
Sta. Maria, Zamboanga City, Philippines, after being duly sworn to oath in accordance with law,
hereby depose and state :

That I am the driver of the following described motor vehicle, owned by ALEXANDER
G. CUYUGAN, a resident of Veterans Ave., Zamboanga City, to wit:

ISUZU Mux Wagon, with Engine Number-4JK1MGO101, Chassis

Number-MPAUCR86GET000433, and Plate No. AAF 6387, covered by
Certificate of Registration No. CR-199878464, dated 10/14/2014 and Official
Receipt No.610850785, dated October 14, 2014, both of the Land Transportation
Office, Zamboanga City District Office;

That last September 11, 2016 at around 10:30 in the morning, that upon my return to my
residence at Sta. Maria, this city, I was an able to avoid the fence of our neighbor, I accidentally
hit it, the motor vehicle was damaged, particularly the following damage, to wit :

1. Right side rear door;

2. Right quarter panel;
3. Right rear bumper;
4. Right Side rear end step board;

That I am executing this affidavit to attest to the truth of the foregoing narration of facts
and for whatever purpose this may serve.
