Use this working at height Risk Assessment checklist as a template to identify any working at height (fall) hazards and
as a last minute risk assessment before starting your work at heights.

Have potential or existing (fall) hazards been identified?

Do you understand your work instructions on how to work with fall protections systems?

Are (fall) injury prevention systems required and in place? (F.e. trauma relief straps)

Will a fall will be arrested before hitting the ground or other lower level or structure?

Is there a safe access point (free of obstructions) to the work area or jobsite?
Stairs, walkways, ladders, mechanical lifts.
People required to work at height have been provided with adequate information,
instructions and training for before work is being performed.

There is no risk of falling objects striking people below.

Is there a fall protection rescue plan established and available for all involved?

PPE

Is all necessary PPE available for this job? Proper shoes, hard hat, safety vest, harness, etc.

All fall protection PPE is checked before use.


A visual inspection for and tears, cuts, deformations or other damage.
The date of when your harness was last inspected is NOT more than a year ago
(If not, don’t hesitate, don’t start working!)

Ladders

Is there a safer, more appropriate, mean available instead of using a ladder?


For example a platform ladder, scaffold or (mobile) elevated work platform.

When working on the ladder: can three points of contact be maintained at all times?

Permanent Fall Protection

Has all equipment, such as anchorage points and access methods,


been inspected within the last year? (If not, don’t hesitate, don’t start working!)

Is the proposed fall protection system able to carry the loads for this job?
