JOINT AFFIDAVIT OF COHABITATION

WE, RODOLFO SALAZAR y CAMAY, born on 30 August 1980 in San

Juan, Balagtas, Bulacan and MARISSA D. DOBLAS, 12 December 1973, both
Filipino, single and residents of Brgy. Caingin Patubig, Bocaue, Bulacan, after having
been duly sworn to in accordance with law, hereby depose and state, that:

1. We have been living together as husband and wife since continuously and
without interruption since 2007 up to the present or for a period of more than five (5)
years;

2. During our cohabitation and even until the present we remain both of single
status and therefore there is no legal impediment for us to marry each other;

3. We are blessed with four (4) children namely:

pursuant to the provision of Article 34 of the Family Code of the Philippines.
