SWORN STATEMENT

I, FAIRY GUARD JR., 40 years of age, single, currently residing in Lot No.
50, Katarungan St, Brgy. Itaas Antipolo City, after having been duly sworn
to in accordance with law, do hereby depose and state that:

1. I am a security guard under Magtanggol Integrated Security

Services Corp. for 15 years;

2. That I am assigned at Parking area of the Forest Ridge Village for

the month of July, August and September 2018;

3. That I am the security guard assigned on the evening of 15 August

2018 at Parking area of the Forest Ridge Village;

4. That MR. TATAY N. BEAUTY’s carousel entered the parking area

with Plate Number 8888 BEAU on the evening of 15 August 2018;

5. That the abovementioned vehicle is still parked in the parking

area of the Forest Ridge Village;

6. That I am executing this Sworn Statement to attest to the truth of

all the foregoing statements and for whatever legal purpose it may
serve.
