 A fully completed application form INZ 1189 or INZ 1017
 Two recent passport-sized color photographs, (must be less
than 6 months old )of each person included in the
application (any background)
 Valid passport(s) or copy of the passports of each person
included in the application

NON REFUNDABLE regardless of the outcome of your
application
 Formulir Aplikasi INZ 1189 atau INZ 1017 yang telah diisi
lengkap
 Dua lembar pasfoto terbaru (harus kurang dari 6 bulan)
berwarna, ukuran paspor, untuk setiap orang yang termasuk
dalam aplikasi (latar belakang bebas)
 Paspor yang masih berlaku atau fotokopi paspor yang
dilegalisir untuk setiap pemohon

sponsored by a NZ citizen /resident
 Work Reference letter and leave approval from the
employer or evidence of company’s ownership (for owner
of a company) or student’s ID
 Consent letter for child under 17 years old not accompanied
by both parents and copy of parents’ ID cards (if
applicable)
 Kartu Keluarga (for Indonesian citizens)
 Flight itinerary
 Processing time: approximately 7 working days. Please
note that this is an estimate only and is subject to change
depending on the type of visa application and/or whether
more information is needed to support an application.

 Formulir “Sponsorship Form No. 1025” yang telah dilengkapi
oleh warga negara/resident NZ apabila kunjungan anda
disponsori oleh mereka
 Surat Referensi Kerja dan surat ijin cuti atau bukti kepemilikan
perusahaan (untuk pemilik perusahaan) atau Kartu Pelajar
(untuk pelajar dan mahasiswa)
 Surat persetujuan dari kedua orang tua untuk anak di bawah
17 tahun bepergian tanpa kedua orang tua dan fotokopi KTP
kedua orang tua.
 Kartu Keluarga (untuk warga negara Indonesia)
 Jadwal penerbangan
 Waktu pemprosesan: sekitar 7 hari kerja. Harap diperhatikan
bahwa waktu proses ini hanya perkiraan saja dan dapat
berubah tergantung pada jenis aplikasi visa dan / atau apakah
kami perlu informasi tambahan untuk mendukung aplikasi
anda.
