Narrative Report

Pursuant to Division Memorandum No. 0178, s. 2019, our school has organized a school
watching team comprising of 20 student-members from different level and organization of the
school. The undersigned was designated by the principal to be the moderator of the said team
to facilitate a student -led school watching and hazard mapping activity. The organization and
preparation of the team was made possible last June 6, 2019, Thursday.
June 7, 2019, after organizing committees, the School Watching Team has conducted a
so-called Student-Led School Watching and Hazard Mapping Activity wherein members of the
team will roam around the campus to observe classroom buildings and areas to identify
hazardous stuff using the School Watching Checklist.
The Student -Led School Watching and Hazard Mapping are initial steps in engaging
learners in disaster preparedness and management. This activity will raise awareness and equip
learners with information and skills necessary for addressing the impact of hazards.
The objectives of the said activity are the following:
 Improve students’ level of awareness on the potential hazards and risks within and
around schools;
 Increase the participation of children in addressing potential disasters by reducing
vulnerabilities; and
 Inculcate a culture of safety in the day-to-day experience of children in school.

Attached in this report is the consolidated result of the conducted survey.

Warmest Greetings!

I would humbly like to request for permission to conduct the Student-Led School Watching and
Hazard Mapping Activity which will be held on June 7, 2019 as per DepEd Order No. 23, s. 2015.
This activity aims to instruct all public elementary and secondary schools in the specific
engagement of learners in identifying and addressing hazards and risks.

The Student -Led School Watching and Hazard Mapping are initial steps in engaging learners
in disaster preparedness and management. This activity will raise awareness and equip learners
with information and skills necessary for addressing the impact of hazards.

The objectives of the said activity are the following:


 Improve students’ level of awareness on the potential hazards and risks within and
around schools;
 Increase the participation of children in addressing potential disasters by reducing
vulnerabilities; and
 Inculcate a culture of safety in the day-to-day experience of children in school.

I intend to conduct the activity within the school on the said date at 3:30 in the afternoon.

Attach herewith is the list of Student-Led School Watching and Hazard Mapping participants.

May I also request your good office that the attached list of Student-Led School Watching and
Hazard Mapping participants would be excused from their classes of their respective advisers
and subject teachers.

Consolidated Result of the Conducted Survey

The student-led school watching and hazard mapping are initial steps in engaging
learners in disaster preparedness and management. This activity will raise awareness and equip
learners with information and skills necessary for addressing the impact of hazards. Through this
activity, the Department of Education (DepEd) creates an opportunity for learners to
understand their role in disaster risk reduction and management (DRRM).
Mentioned below are the identified hazard factors and /or at-risk areas in the school
based on the observation of the School Watching Team during the student-led school watching
and hazard mapping activity held last June 7, 2019.
 Slippery pathway
 Flooded area
 Exposed chemicals and liquids
 Lack/ absence of storage for equipment
 Dripping ceiling
 Open/ clogged canals
 No ramps for elevated school buildings or other facilities
 Broken/ dilapidated ceiling
 Presence of stray animals inside the school campus
 Unmounted cabinets/ shelves
 Trip hazard
 Falling hazard
 Hazardous area
