REQUIRMENTS FOR THE PROSPECTIVE BIDDER PASS FAIL

1. Eligibility and Technical Components
Class “A” Documents
Legal Documents
1. Registration Certificate from the Securities & Exchange Commission
(SEC), Department of Trade & Industry (DTI) for SOLE proprietorship, or
Cooperative Development Authority (CDA), or any proof of such registration.
2. Mayor’s Permit issued by the city or municipality where the principal office
of the business of the prospective bidder is located
3. Tax clearance, per EO 398, s. 2005 as finally reviewed and approved by the
BIR
Technical Documents
4. Certificate of PhilGEPS Registration
5. Statement of the prospective bidder of all its ongoing government and private
contracts, including contracts awarded but not yet started, if any, whether
similar or not similar in a nature and complexity to the contract to be bid,
within the relevant period as provided in the Bidding Documents, and
6. Statement identifying the bidder’s single largest completed similar to the
contract to be bid; except under conditions provided for in Sec. 23.5.1.3 of the
IRR, within the relevant period as provided in the PBDs
7. “Authority of the Signatory” of the prospective bidder/proprietor of the
business through a “Special Power of Attorney”; authorizing the
representative/s to transact and sign in on his/her/their behalf.
8. Organizational chart for the contract to be bid
9. Sworn Statement in accordance with section 25.2 (b) (iv) of the IRR of RA
9184 and using the form prescribed in Section IX. Bidding Forms.
Financial Documents
10. Audited financial statements, showing among others, the prospective total
current assets & liabilities, stamped “RECEIVED” by the BIR or its duly
accredited and authorized institutions, for the preceding calendar year which
should not be earlier than two (2) years from the date of bid submission.
11. NFCC computation or CLC in accordance with ITB Clause 5.5
Class “B” Documents
12. Bid Security as prescribed in ITB Clause 18 Bank Guarantee/Bid Security
2. Financial Component
1. Financial Bid Form (includes bid prices and bill of quantities as well as
Applicable price schedule

The Bidder is responsible to double check the full description of above requirements in the Bidding documents issued by the
Procuring Entity.

Prospective Bidder shall submit duly accomplished forms as specified in the Bidding Documents in two (2) separate sealed envelopes
t to the BAC Chairman. The first envelope shall contain the Technical Components of the Bid, which shall include the eligibility
requirements; the second shall contain the financial component of the Bid.
