Introduction: The DepEd’s Vision and Mission encompasses


the ultimate goal of our educational system to every
learner. Thus, with these we are inspiring our
learners to become productive citizens of the
country. We envisioned that all of them could take
part in nation building and development.
Sta. Fe National is an institution that inculcates
God- fearing, highly competitive and lifelong
learners gearing towards excellence. The school
provides basic quality education through values
formation, effective instruction, with competent
teachers, school administrator and committed
stakeholders.

Poor academic performance of students

Based on the MPS result submitted by the
teachers in every grading period and Report Card
Form 138 A of the students

The students themselves and the teachers

teaches facilitate better delivery of the lesson
and make learning accessible to all.

Key Changes in my school as a I envisioned that after the implementation
result of this project of this project the Conditionally Promoted Students
will be able to cope and pass all the learning areas.
Thus, help them as well to acquire the
competencies and the KSA’s in all subjects.

Describe current situation The Promotion Rate for school year 2017-
(problem or opportunity) in your 2018 is 85. 45% only. This rating is lower compared
school that you need to address to other Performance Indicators. We have
through your project Conditionally Promoted Students (failed in one or
two learning areas) both in Junior and Senior High
School.
Some of our students particularly the Senior
High School learners engaged into having part time
work and earning their own money to finance their
studies. It is one the reasons that they missed their
classes. Another reason as well is that we have
commuters from the neighbouring barangays (Sta.
Cruz and Libertad). During rainy days some of
these students absent from their classes and just
stay at home. These were the same students
identified as potential failures as reported by the
class advisers.
With this PROJECT CHANCE, the students
will be given schedule for their remedial classes
through Modules and SIM’s. Thus, helping them to
cope with their grades and pass in all learning
subjects.
