Abstract
fragment, its location, the degree of curvature of the root canal, degree of visibility,
possibility to create a straight line access to the fractured instrument,
morphological characteristics of the dental root, clinician’s experience and
endowment. The treatment options consist in removing the fragment - the ideal
solution - by dedicated instruments and kits, including ultrasonic means; bypass -
may endanger the strength of the root, favoring the fracture of other instruments;
root canal instrumentation and filling coronal of the separated fragment - not
recommended; surgical methods, depending on the location of the fractured
fragment. Removal of a separated instrument is one of the most difficult
therapeutic approaches, requiring specific training, technical endowment and
advanced techniques, is time and resource consuming and involves several risks:
fracture of other instruments, ledging, perforations, cracks or root fractures by
weakening of the root walls. Success depends on the selection of the optimal
method relative to the respective clinical case, the key being to correctly evaluate
the benefit / risk ratio.
