LOCK OUT PROCEDURE

OBJECTIVE

To ensure the safety of persons working on electrically power driven machinery or electrical apparatus by
preventing accidental switching in.

4.8.1 LOCK OUT PROCEDURE

1. HIGH TENSION

a. No work shall be conducted on any high-tension equipment before it is isolated at the power supply.
Isolation shall be achieved by switching off the main circuit breaker, racking the circuit breaker out of
the switchboard shutters or the switch locking mechanism.

b. After lockout has been completed the system shall be tested for voltage at the points to be worked on
using test sticks.

NOTE: FIRST TEST THE TESTING STICKS TO ENSURE THAT THEY ARE IN WORKING
ORDER

c. After establishing that the system is indeed “dead”, shorting and earthing clamps shall be fitted
to the supply side at each of the points where the work is being done. It is the responsibility of the
persons applying shorting devices to remove them before removing their own lock from the
lockout. The reason, time and date of LOCKING OUT must be recorded in the sub station book.
Before switching in it must be established that it is safe to do so and logged in the logbook with
the date, time, names and signature of the responsible person.

2. LOW TENSION

a. When isolating a low tension supply, the circuit breaker must be switched off and the handle of the
circuit breaker locked by the person responsible for isolation. The circuit breakers will normally be
on the main control panels and be clearly marked with the correct equipments name or number.

NOTE: DO NOT LOCKOUT AT THE STOP / START SWITCH OR COVEYOR TRIP WIRE

b. If, for any reason, the circuit breaker cannot be locked when isolating, the electrician will disconnect
the cable from the circuit breaker feeding the apparatus or equipment being isolated.

LOCK OUT PROCEDURE


GENERAL

a. When more than one person is carrying out work on separate parts of the isolated circuit each such
person will witness the lockout and apply his own lock to the lockout. A lockout scissors may be
used to add more locks if required.

b. When unskilled labour is assisting with work on machinery and equipment, the Foreman will supply
his personal lock which he will not remove until he is satisfied that all persons involved in the work
are clear safe and accounted for.

c. All “off mine” personnel such as servicemen or contractors will be issued with a lock and a copy of
the lockout procedures. These must be acknowledged and signed for by any such person.

d. The person responsible for the work being done will do locking out and unlocking personally. He
will retain the lockout key at all times.

e. In addition to the lockout procedure, notices bearing the artisans’ name and trade as well as “DO

applying his lock.

f. All scraper winches will be equipped with switch gear which can be locked out with a padlock by the
appointed winch driver when it is necessary for him to leave the winch. He will whenever working on
the scoops, snatch blocks or ropes ensure that his winch switch button cover is locked.

GENERAL

g. The winch driver at the end of the shift will ensure that the winch is switched off. Switch button cover
is locked and his keys are handed to his Team Supervisor.

h. The Stoping / Development Team Supervisor will ensure that keys to open locks are only issued to
Appointed winch drivers.

i. The Shift Supervisors will follow up on this procedure in their sections and log the results of their
inspections for at least one winch daily.
