SIR:

This is a request to respectfully allow the undersigned to take a special quiz to compensate for
his missed quiz held last November 14 2018 on the subject LAW ON PARTNERSHIP AGENCY AND
TRUST.

It is admitted that the undersigned was not present during the day of the quiz, but his absence is
without of ill motive. On the day of the quiz, the undersigned had a fever and was experiencing
serious light-headedness with dizziness making it impossible for the undersigned to either drive
or commute. The serious light-headedness with dizziness that the undersigned was suffering on
that day is not that of which is common to fever, but is more related to his current medical
condition of having high blood pressure and very high triglycerides.

The above mentioned quiz has materially affected the grade of the undersigned leading to his
failure. It is to be noted that failure from this subject may cause any student to be delayed or
extended for a year however this situation may still be prevented should this request be granted
favourably. Attached herein is a copy of his medical record subject for your viewing. Hoping for
your kind consideration.
