LOGBOOK REVIEW

Overview
The form can be used at any time but the IRSE requires evidence of a review at least once a year. When an assessor reviews the logbook for renewal,
surveillance or initial issue of a licence then this form should also be used to formally register that the logbook has been reviewed by the assessor and act as
a record of assessment. It is not acceptable just to tick or say OK next to each section; the reviewer must state briefly why they feel the requirements for the
section have or have not been met.

Completing the logbook review form


As part of the surveillance process licence holders must have their work experience verified on an annual basis. At the five year point the competence
assessor will need to see evidence that the review has been undertaken annually and that any identified deficiencies have been noted and corrected. IRSE
have provided a typical form that can be used for this purpose, but similar forms currently used by organisations that contain the same information can still
be used. At the time of surveillance the assessor will wish to see evidence of four annual reviews covering the intervening years since the issue of the
licence.

There is a row for each section of the logbook which must be reviewed and a brief comment made (a tick is not acceptable). Where the section does not
meet the current IRSE requirements then an action plan with close out date should be agreed with the licence holder and a follow up review planned to
ensure the action is completed. Guidance on how to fill in the forms, together with an example, is provided for each section. In addition reviewers should
note the following:

Obligations of licence holders


The licence holder has agreed to comply with the requirements of the Licensing Scheme as published in the Obligations of Licence Holders. This was
issued to them with their licence and the latest issue of this document can be downloaded from www.irselicensing.org . All reviewers should ensure that the
licence holder has the latest issue in their logbook and that they have read and understood it.

Complaints and endorsements


Reviewers should ensure that the latest version of the complaints procedure and record of complaints form are in the logbook and that licence holder is
aware of the process as explained on these pages (latest version can be checked at www.irselicensing.org ).

Work experience forms and witness status list


Reviewers should ensure that those signing the work experience forms after 1/10/15 have also entered their names and details on the witness status list.
