Overview
The form can be used at any time but the IRSE requires evidence of a review at least once a year. When an assessor reviews the logbook for renewal,
surveillance or initial issue of a licence then this form should also be used to formally register that the logbook has been reviewed by the assessor and act as
a record of assessment. It is not acceptable just to tick or say OK next to each section; the reviewer must state briefly why they feel the requirements for the
section have or have not been met.
There is a row for each section of the logbook which must be reviewed and a brief comment made (a tick is not acceptable). Where the section does not
meet the current IRSE requirements then an action plan with close out date should be agreed with the licence holder and a follow up review planned to
ensure the action is completed. Guidance on how to fill in the forms, together with an example, is provided for each section. In addition reviewers should
note the following:
