I, have been directed to in your attention to the subject mentioned above and to state

that the Government has decided empanel the following new private hospitals for providing

rleatment to the Haryana employees/pensioners and their dependents:-

l. To provide seam.less and ftee sewices to Haryana Golt. Employee/Pensioners and

their dependents, the orivate hospitals shall extend all normal courtesies and

facfitate the admission, and discharge of Haryana Gort. beneficiaries with no

delay or denial of medical

4. A Nodal Officer shall be ted at level of empanelled private hospital. The Nodal
will communicate with the Civil Surgeon or/ and deparrment in case of any

gnevance.

5. In case of any emergency/ /overflow of patients in Gor,t. Hospital during

epidemics the empanelled ital will agree to share their ambulances, mortuary ,

ICU/CCU , burn unit, ward etc. in need based manner.

6. AII skrutory Rules/G Acts/Notifications issued by State Govt. from time to

time (As already mentioned the application form for New Empanelment Policy) shall
be followed by the hospitals. The same shall be applicable on existirg private

empanelled hospitals.

7. The hospital will abide to all National Proerammes and its related IEC activities. The
data needed for National or Health Indicators must be shared with Health
Department. The same shall applicable on existing private empanelled hospitals.

8. The hospital shall follow bfuth & death guidelines issued by the State Government
from time to time. Any developed by the Govt. will have to be adopted by the
hospital.

9. It will be the of the hospital to provide accreditation certi-ficate and

fu rther renewal certificate before the expiry date.

10. Guidelines already framed State Gort. policy dated 21.05.2015 for empanelled
hospitals shall be app on new empanelled hospitals. However, when New
Packages are made {s or sewices are started in Cashless mode new guidelines shall
be issued for compliance consent of concerned hospital

The above listed hospitals remain empanelled for period of three years subject ro
validity of accreditation Thereafter, the hospital will apply afresh.

12. Heaith Departrnent reserves the right to call for record of rrearment of any parient from
any empanelled hospital ro no mal-practise takes place.

13. The reimbursement on incurred on non package rreatment will be allowed

at PGI Chd. Rates"75% of balance amount.

14. All empanelled hospitals maintain record of the treatment of the patients.
t5. Ordinary no relaxation the policy shail be given. For the convenience of the
employees/pensioners and dependents however, relaxation can be granted in
exceptional cases will Prior of Hon ble Chief Minister and Finance Departmen[.

Such relaxation can be in the conditions like rreatment of a very old person,
highly sensitive narure of involving multi-system disorder and rrearments taken
in life threatening siruadon ecc. Juscification for such expendirure, however,

shall have to be first ob from a committee of expefs headed by DGHS and rwo
subject specialists posred in or PGIMS, Rohtak.
