Dear faculty members,
This is a reminder of the process for submitting course portfolios for all sections faculty members teach. In line with KSU's approach, COLT's Quality Unit aims to maximize the use of technology in terms of e-forms. Hence, the contents of course portfolios must be uploaded electronically via the link below. The procedure to submit the portfolio will be as follows: 1. Upload the contents of the course portfolio through the following links: a. Course portfolio for all courses: https://goo.gl/oxdUkg b. Course portfolio for the Project: https://goo.gl/2yGzwK Ø Save and resume function is active now. This enables the faculty to save their work and resume uploading the files later. The portfolio will be sent only by clinking Submit. 2. NCAAA new forms (2015 version) must be used, forms other than this version won’t be accepted. (Attached is NCAAA forms – 2015 version) 3. Faculty members have to be accurate when filling in all the forms. (portfolios will be reviewed and will be reported to the college Dean and Vice Dean) 4. In cases where two or more faculty members teach the same course, a combined report must be submitted. This applies as well to cases where more than one section is taught by one faculty member. 5. In regards to sample students' exams, 3 of each exam are required - 9 in total - (3 samples of the first midterm, 3 samples of the second midterm and 3 samples of the final exam). 6. Results statistics must be submitted (these are easily retrieved online once results are submitted via edugate). 7. Scans must be clear. We highly recommend you to use Genius Scan (a free mobile app to scan your documents – click or scan the attached barcodes below to download the app). 8. Complete requirements and content portfolios will be honoured with a certificate of thankfulness from the vice dean. The criteria are: a. The submission should be before Sunday 05th February 2017. b. The portfolio must contain the required files and the content of the files has to be complete. c. Scans must be clear.
