2. Bid Price

2.1 The contract shall be for the full quantity as described above. Corrections, if any, shall be made by
crossing out, initializing, dating and re writing.
2.2 All duties, taxes and other levies payable by the contractor under the contract shall be included in
the total price.
2.3 The rates quoted by the bidder shall include the cost of inland transportation and other incidentals
for delivery of the goods to the final destinations namely
a) Unloading, safe storage and handling of consignment, delivery to the consignee.
b) GST shall be extra as applicable.
2.4 The Prices should be quoted in Indian Rupees only.
2.5 Each bidder shall submit only one Price bid.

4. Validity of Price bid:-

Price bid shall remain valid for a period of 60 days after the deadline date specified for submission.

5. Evaluation of Price bid:-

5.1 The Authority will evaluate and compare the Price bid determined to be substantially responsive i.e.
which

(a) Are properly signed; and

(b) Conform to the terms and conditions, and specifications
(c) Not furnishing Catalogues, Point wise compliance statement of

Technical specification, and producing the sample of the quoted item within one week note, if called for
would be treated as a violation of the terms of the tender and the bids would be treated as non
responsive.
5.2 Authority reserves a right to increase or decrease the quantity of printing material by 10 %.

6. Award of contract:-
The Authority will award the contract to the bidder whose Price bid has been determined to be
substantially responsive and who has offered the lowest evaluated Price bid price.

6.1 Notwithstanding the above, the Authority reserves the right to accept or reject any Price bid and to
cancel the bidding process and reject all Price bid at any time prior to the award of contract.

6.2 The terms and condition in this RFP are confidential between the service provider and the authority,
shall not be disclosed to anyone else, any disclosure violation shall be deemed a material breach and
lead to cancellation of empanelment with NHM U.P.

6.3 The bidder whose bid is accepted will be notified of the award of contract by the Authority prior to
expiration of the Price bid validity period. The terms of the accepted offer shall be incorporated in the
purchase order.

6.4 For delay in supply of the goods beyond the stipulated delivery period, liquidated damages at 0.5%
per week or part thereof of undelivered portion of the contract, subject to a maximum of 10% of the
contract value is leviable. If the service provider fails to provide the deliverable in stipulated timeline
then, the L-2 bidder will be considered for the work.

6.5 The Authority will pick up random samples from the delivered printed material and send it to
Rajkiye Mudran Karyalaya, Allahabad to examine the correctness of Technical Specifications. The quality
of GSM shall be according to the BIS standards. If the goods fail the test, no payment shall be made to
the supplier. The sample test charge if any will be paid by the bidder.

6.6 You shall furnish performance security for 10% of contract value by means of demand draft drawn
in favour of State Health Society, Uttar Pradesh, payable at Lucknow which will be returned after
satisfactory completion of delivery of the printed Material60 days from the award of contract. The
performance security shall remain valid for a period of sixty days beyond the date of completion of all
contractual obligations by the firm.
6.7Payment Terms-

Payments shall be made upon receiving successful sample quality report and delivery of the goods, 80%
payment against submission of bills and 20 % payment on verified certification from the consignee for
satisfactory completion of supply.
TDS would be deducted on total payment (as applicable)

6.8 Sublet-
The work order hereby shall be assigned to the service provider, shall not enter into any sublet to any
third party.

6.9Jurisdiction-

The Bidding Process shall be governed by, and construed in accordance with, the laws of India and the
Courts at Lucknow, Uttar Pradesh shall have exclusive jurisdiction over all disputes arising under,
pursuant to and/or in connection with the Bidding Process.

Secretary, Medical, Health & Family welfare, GoUP shall be an arbitrator, for any disputes between both
of the parties (Authority & Service Provider).

7. You are requested to provide all the information sought in the Price bid and upload the same on the
portal i.e. http://etender.up.nic.in as a part of its online submission of Bid. The bid preparatory charges
shall be bear by the service provider itself. The Authority will evaluate only those Bids that are received
in the required formats and complete in all respects on or before dd/mm/yyyy …. At 1.00 pm.

8. Online uploaded Price bid will be opened at ……….PM on……………in the presence of bidders.

9. Samples/ drafts/Layout of printing material can be seen or provided at the concerned section RBSK
cell of NHM UP. Designing/printing job shall be submitted at the concerned RBSK cell of NHM UP.
