Dear Sir,

May I respectfully request for a reconsideration on the late filing of my application for
the renewal of my ACR I-Card that expired last February 27, 2018.

I am now 77 years old, sickly and because of my old age poor health condition, I forget
about my ACR I-Card renewal, I apologized for this late filing of my application for the
renewal of my ACR I-Card, that was just realized already expired upon intention to pay my
annual report which I was not able to comply earlier, hence, I am willing to pay my fines and
penalties and request for a reconsideration for the renewal of my ACR I-Card for my residency
in the country.

Hoping that your Honorable Office would grant reconsideration of my above request
for the late filing of the application for the renewal of my ACR I-Card be granted, to maintain
my residency in the country.
