II. Objectives: At the end of the lesson, students are able:
1. To give communicative responses to patient’s complaints. 2. To ask about the dimensions of symptom.

Students have to be able to make questions in order to achieve the objectives above. IV. Nursing note: Nurses have to be able to explore the patient’s complaints by asking a certain questions about dimensions of symptoms. The patient’s responses are reported in the nursing documentation, then it is reported to the physician on duty. Useful expressions (Nursing note: Guidelines for asking about the dimension of a symptom). 1. Location in the body : describe the exact location. 2. Quality/character/intensity : describe in patient’s terms; e.g a person having myocardial infarction (heart attack) might describe the chest is being “squeezed in a vise”. 3. Quantity/severity: chart the intensity of the symptoms(i.e. mild, moderate, or severe) Use a scale of 1-10 for pain, with 10 being the highest. Indicate the degree of impairment; and the frequency, volume and size or extent of the sign or symptom. Note the number of times the patients has vomited, amount each time, and whether nausea is constant or intermittent. 4. Chronology/Timing: note the sequence of development: - Time of onset of the sign or symptom. - Duration (minutes, hours, days) - Pattern of variation and frequency and the course of the signs or symptoms (e.g. do they stay the same, get better, or get worse over time?) - Another question: When do you get the pain? - Follow up question: Does t start in one place and spread? 5. Setting : where is the patient (e.g. at home, in bed, in the car etc), what is the patient doing (e.g. running, sleeping, or eating), and who is the patient with (e.g. mother, spouse, friends) when the symptom occur? 6. Aggravating or alleviating factors: what makes the signs or symptom worse and what makes them better? 7. Associated manifestations: signs and symptoms rarely occur singly. For instance, does the patients have nausea before vomiting? Has there been a weight change since the onset of vomiting? Kualitas / karakter / intensitas: menjelaskan dalam hal pasien; misalnya seseorang yang memiliki miokard infark (serangan jantung) mungkin menggambarkan dada sedang "diperas catok". 3. Jumlah / keparahan: grafik intensitas gejala (yaitu ringan, sedang, atau berat) Gunakan skala 1-10 untuk nyeri, dengan 10 adalah yang tertinggi. Menunjukkan tingkat kerusakan; dan frekuensi, volume dan ukuran atau luasnya tanda atau gejala. Perhatikan berapa kali pasien telah muntah, jumlahnya setiap kali, dan apakah mual yang konstan atau intermiten. 4. Kronologi / Waktu: perhatikan urutan pembangunan: - Waktu onset tanda atau gejala. - Durasi (menit, jam, hari) - Pola variasi dan frekuensi dan tentu saja tanda-tanda atau gejala (misalnya apakah mereka tetap sama, lebih baik, atau lebih buruk dari waktu ke waktu?) - Pertanyaan lain: Ketika Anda mendapatkan rasa sakit? - Menindaklanjuti pertanyaan: Apakah t mulai di satu tempat dan menyebar? 5. Tempat: di mana pasien (misalnya di rumah, di tempat tidur, di dalam mobil dll), apa pasien melakukan (misalnya berjalan, tidur, atau makan), dan siapa yang pasien dengan (misalnya ibu, pasangan, teman-teman ) ketika gejala terjadi? 6. Memperparah atau faktor meringankan: apa yang membuat tanda-tanda atau gejala lebih buruk dan apa yang membuat mereka lebih baik? 7. Associated manifestasi: tanda dan gejala jarang terjadi secara tunggal. Misalnya, apakah pasien mengalami mual muntah sebelum? Apakah ada perubahan berat sejak awal muntah?
