Machines don’t make mistakes, they are compelled to do so by erratic
human behavior and MD variations, Humans are supposed to master m/c & Process but due to non application of mind to foresee consequences of their actions, negligence, fatigue, carelessness, lack of sense of belongingness, everyday conflicts, frustrations, low motivational level, stress etc.etc. make errors and compel machine to make mistakes or system to runaway from their expected line giving birth to accidents /disasters/losses /low productivity. Investigations of past accidents even, near miss clearly indicate behavioral lapses on the part of employees as the root cause.
