I, JUANITO SANDOVAL JR., of legal age, with postal office address at Rm. 419 4th floor, champ Bldg. Bonifacio Drive,
Brgy. 650 Port Area, Manila, after having duly sworn to in accordance with law do hereby depose and say: that,

1. I am the employee of BRILLIANTRADE INC. Last April 2019, ZIM INTEGRATED SHIPPING SERVICE INC. issued in
our favour an Equipment Interchange Receipt with Bill of Lading No. GOSUNGB9358007 with the
understanding that our company should surrender the original receipt if we intent to get back the said
container deposit.

2. I am the custodian of the said container deposit receipt. On April 2019. I lost the original Equipment
Interchange Receipt and despite my efforts to look whereabouts.

3. For which reason, I checked all my places in my house and also in our cars but still failed to locate the said
Equipment Interchange Receipt. I tried to ask all the people in my house but all of them didn’t know where
the Equipment Interchange Receipt is.

4. After my diligent effort searching for the Equipment Interchange Receipt, my effort went for naught.

5. I execute this affidavit for the purpose of attesting that I have lost the Equipment Interchange Receipt and in
exchange, ZIM INTEGRATED SHIPPING SERVICE INC. Will release the Equipment Interchange Receipt in our
favour and for any other legal purpose it may serve.

6. Upon receipt of the Equipment Interchange Receipt, me and our company, and the consignee, waive any
claims against ZIM INTEGRATED SHIPPING SERVICE INC. and forever release it of any liability as far as the
payment/ return of the container deposit above-mentioned. That if and when the original of the container
deposit receipt will eventually be found, it cannot be used by our company and/or any of its representatives
to collect the same.
