NO Issuance of Provisional Authority (PA) every Monday

COVERAGE:

- All received TNVS application or application with already assigned CASE

NUMBER
- The appointment will only cover transactions for issuance of Provisional
Authority (PA), other TNVS concerns will be entertained by the LTFRB
Public Assistance & Complaint Desk (PACD)

REQUIREMENT:

- Bring the print out of the PA Appointment Confirmation Email

- Only applicant/operator is allowed to receive the PROVISIONAL
AUTHORITY
- Special Power of Attorney will be allowed only to direct descendant or
Ascendant
- Bring the original copy of Application with Receipt of Payment, Motion
for Extension of PA if the previous issued PA is Expired.
- Personal Passenger Accident Insurance
- Proof of Garage ( for applications received from JULY 2019 to present)
o Sketch of location of Garage
o TCT or Tax Declaration of the garage (under the name of the
applicant)
o Lease Contract or Affidavit to Use as Garage, or Agreement (with
TCT or Tax Declaration under the name of the Lessor or owner of
the garage)
