Stage Play (Filipinas 1941) at La Salle University, Bacolod City on March 13,2015,Friday, 1:00 P.M.

It is understood that outmost care, guidance and supervision will be given to my son/daughter.
However, any untoward incident that may happen to our son/daughter beyond the control of the teacher and the
school,the said teacher and the school will not be held responsible for it.

I further expressly waive my right to file any legal action and free authorities from any liabilities arising
thereto.
