Chief Complaints:
Mr' Rajeev Udaiwal a 47 yrs' old gentleman presented with complaints
has been admitted here
of pain in abdomen for 2-3days. He
for evaruatlon and further managernent.
General Examination :
On evaluation patient was found to be conscious
cooperative, oriented to time, place and person. patient
was afebrile. Per Abdomen tenderness present.
Liver and spreen not palpable, bowel sound present.
Systemic Examination :
His Heart Rate was Regular at 78/min' with Blood
Pressure of 110/g0 mmHg. Heart Sounds
normal. B/L Air Entry equal. 51, 52 were

Past History :
Medical - He is hypertensive and non_diabetic.
lurgical - No significant history of any surgery in the past.
Family - Patient has no family history of caq
aiuu"tes, hypertension.
Allergy:
No significant allergy to any food material or drugs.
Personal Habits :
Patient is a non-smoker and he does not consume
tobacco or alcohol.
Course in the Hospital :
Mr' Rajeev Udaiwal, was admitted and evaluated. on investigation
his cBC showed Low Hb (11.6). And
his SGOT & SGPT were raised' along with raised Sr.
Amylase and Lipase. His usG abdomen and cr
abdomen showed Bulky pancreas with fat stranding.
He wsus managed with I.v antibiotic, ppl, IV fluids,
analgesic and other supportive treatment. After
stabil izationhe is now being discharged on medication.
