CARDS:

1. Do I need to carry my cashless card when I go to the hospital?


Ideally, you should always carry the cashless card with yourself, when getting admitted to the
hospital. But, in the event that you do not have the cashless card, you should get in touch with the
Vantage representative who will help you to provide card no and policy no.
However, it is advisable to carry a valid photo identity proof of the patient (Employee ID Card,
Driving license, Election card or any card which is approved by Government of India), irrespective
of whether you are carrying the cashless card or not.

2. What if I have not got your cashless card yet? Am I covered? What do I need to do to get
cashless treatment?

The claims would be settled without the cards provided the claimant (the employee or the
dependent) is endorsed in the policy. You would be entitled to cashless treatment but in such
case you are requested to get in touch with your Vantage representative before the
hospitalization.

3. Post my marriage, my surname has been changed; however, my insurance card has my
maiden name written on it. How do I get the name changed on my card?

You need to intimate Vantage for required correction via Vantage connect -
Under My Enrollment -> a window for correction is available, Please mention the required
correction in the available window and save it. Vantage representative will get the endorsement
passed for the change in surname. You will get the updated card post the endorsem ent is
passed. You may need to submit proof for the change like marriage certificate or an affidavit.

Please note that it is very important that you get an endorsement passed especially when your or
your dependent's full name is changed. The same can be done by intimating your HR/ Vantage
about the same and submitting the newspaper cutting of the announcement of such change along
with the affidavit.

Please ensure that at the time of a claim, the name appearing on the prescriptions and the
medical bills should be same as the one appearing on the health card.

4. My actual age is 36 years. However, the health card shows 42 years. What should I do
now?

If the difference in your actual age and the one appearing on the card is more than 2 years, You
need to intimate Vantage for required correction via Vantage connect -
Under My Enrollment a window for correction is available, Please mention the required correction
in the available window and save it. Vantage representative will get the endorsem ent passed for
the change in Age. You will get the updated card post the endorsement is passed. You may need
to submit a valid age proof like birth certificate, school certificate, PAN card etc. and get the
endorsement passed for the age correction.
