1. Accomplish the form by Grade level.


2. Fill out the needed data by subject area and language available.
3. Column C: Actual Quantity of LRs is the Total Number of LRs received by school.
4. Column D: Losses & Damages is the Number of Lost LRs/LMs and Damages that are no longer Usable.
5. Column E: Usable are LRs/LMs that can still be used by the learners.
6. Column F: Actual Enrolment for SY 2019-2020 is the Enrolment as of July 31, 2019 by Grade Level who will receive the LRs/LMs.
7. Column G: LR Shortage/Deficiency is the Number of needed LRs/LMs (KULANG NA LIBRO).
8. Column H: LR Excess is the Number of Excess (SOBRA) books that are not distributed or used by the learners.
9. Column I: Used - type YES if the LRs/LMs are used or distributed to the learners or NO if the LRs/LMs are not used or distributed to the learners.
10. Column J: Remarks - for your comments, reasons, justifications, issues, concerns and problems.

Note: Usable inventory considerations:


1. Must include only learning materials/textbooks which are still in good and fair condition and can still be issued to students this SY 2019-2020.
2. Must specify each mother-tongue based language/s per learning areas, particulary, for Grades 1 to 3.
3. Must specify the book or area of specialization, especially for Grades 11 and 12.
4. DO NOT ADD new Column and Rows.
5. DO NOT change or make any changes in the templates.

School Reports shall be submitted to District Property Custodians (DPCs) for Consolidation. DPCs will be the one to submit the electronic copies of school
LRSRs at LRMDS.
