resident of Ajinomoto Purok 9, Barangay IX, Victorias City, Negros
Occidental, after having been sworn to in accordance with law, hereby
depose and state the following:

1. I am applying for the position of ____________________________ in 7th

Municipal Circuit Trial Court EB Magalona - Manapla;

2. That I have no relatives within the third degree either of

consanguinity or affinity who is an incumbent employee in the
subject branch;

3. That should the Supreme Court of the Philippines ascertain that I

have relatives in 7th Municipal Circuit Trial Court EB Magalona -
Manapla, the Supreme Court of the Philippines reserves the right
to cancel the processing of my application;

4. That I understand that should I be employed, any

misrepresentation on that matter is a ground for an administrative
case and termination;

5. That this affidavit is executed to attest to the veracity of the

foregoing statements.

1. That I am an applicant for the position of Clerk II of the Presiding

Judge of 7th Municipal Circuit Trial Court E.B. Magalona –
Manapla, Negros Occidental;

2. That in view of the said application, I am manifesting that I am not

related to any existing personnel of the said court up to fourth
degree of consanguinity or affinity, or any level;

3. That I execute this affidavit in accordance to existing law in its

support of such application.
