C. ASSESSMENT PLAN
• Written Test: Refer to Written Test in the Evidence Plan at the end of each activity to check on their learning.
 Performance Test: Institutional assessment with oral questioning and written test in operating an automated information
system.

D. TEACHER’S SELF-REFLECTION OF THE SESSION


At the end of the session, evaluation will be done in order to determine if there are any improvements or enhancement
of the training delivery. Also, recommendation will be given to each trainee with regards to their performance as an outcome of the
Institutional Assessment.
