SEPARATION AGREEMENT

BETWEEN A HUSBAND AND WIFE

This AGREEMENT made at __________________________________________________on this day of

______________ 2019, by and between:

LOIDE MELINDO y Valix, legal age, Filipino citizen, married and a resident of Brgy.
Carino, Paniqui, Tarlac (hereinafter called the “HUSBAND”).

-and-

JENNIE MELINDO y Guinto, legal age, Filipino citizen, married and a resident of
Brgy. San Antonio, Sto. Tomas, Batangas (hereinafter called the “WIFE”).

-WITNESSETH THAT-

WHEREAS, The Parties were married on APRIL 09, 2011 at Darasa, Tanauan City, Batangas
solemnized by Rev. Fr. Virgil F. Fabriquel. However, due to certain irreconcilable differences, the
Parties decided to separate on __________________________________;

WHEREAS, they want to live separate, apart from each other and intend to live separate at
all times unless there is any reconciliation;

-NOW THEREFORE-

1. The Parties are living apart from one another and no party shall have any right
authority over the other or shall institute any legal proceeding for restitution of
conjugal rights or otherwise;

2. The husband and the wife shall jointly pay for the maintenance and support of the
children.
Support comprises everything indispensable for sustenance, dwelling, clothing,
medical attendance, education and transportation, in keeping with the financial
capacity of the family.

3. The child custody shall be subject to another agreement, however, the party entitled
to the child custody and guardianship must maintain and educate the said children
until they shall respectively attain the age of majority.

4. The Parties shall be free to live separate and apart from one another without any
encumbrances of any kind. Neither Party shall interfere with the personal
decisions of the other or attempt to limit the freedoms of the other.

5. This agreement shall be revoked by the death of either the husband or wife.

In Witness whereof, the Parties have caused this SEPARATION AGREEMENT BETWEEN
A HUSBAND AND WIFE to be duly executed and effective as of the effective date _______________ 2019
