I, GLYN B. ARRANCHADO, of legal age, Filipino, single and a resident of Brgy.

Hibulangan, Villaba, Leyte, Philippines, after having been duly sworn to in accordance with law,
do hereby depose and state that:

1. As of the present time, I am not employed in any capacity at any business establishment,
corporation, government and individual person;

2. I have no pending case before any court, judicial or quasi-judicial body and has not been
issued any hold departure order from any government institution or agency;

3. I execute this affidavit to certify and attest to the truth of the foregoing facts and to support
my application to FILE A PETITION FOR CHANGE OF NAME, GENDER, FATHER’s NAME
and for whatever legal purpose/s that this affidavit may serve.
