* Baby Denisse 7months/F admitted with above said diagnosis underwent
glutaraidehyde treated pericardial patch closure of VSO, pulmonary valvotomy. Pericardial
patch plasty of pulmonary artery confluence and RPA, double ligation of PDA. Direct
closure of PFO done on 08/03/2019. She came off bypass at first attempt in complete
heart block ventricular pacing was initialed, she was requiring dobutamine and adrenaline
supports. Shifted to ICU on ventilator in a stable condition on dual chamber pacing
Was extubated within half hour of shifting to the ICU, post extubation respiratory efforts
were good. The inotropes were tapered and stopped, detubed and arterial lines were
removed by POD-2 She remained in complete heart block till 10th POD when it reverted
to sinus rhythm. Her pacing wires were removed on POD-12, post wire removal the echo
was within normal limits. She had a good recovery in the hospital after tins and is getting
discharged on POD-13 in a haemodynamically stable condition.

Large perimembranous ventricular septal defect L-R shunt, discrete subaortic
membrane, right pulmonary artery stenosis, patent ductus artenosus mild pulmonary
valvar stenosis, small PFO, good biventricular function, mild pulmonary arterial
hypertension sinus rhythm
