Chest Pain Differential

Etiologies:

● Chest wall
● Cardiovascular
● Hyperadrenergic states
● Gastrointestinal
● Pulmonary
● Mediastinal
● Psychogenic
● Refferred pain
Chest wall: (36%)

● Muskuloskeletal:
- Costochondritis
- Rheumatic involvement of costovertebral joints
- Non-rheumatic systemic diseases (stress fractures due to coughing; neoplasms;
sickle cell anemia)
● Skin and sensory nerves:
- Herpes zoster
- Post radiation neuralgia
Cardiovascular:

● Ischemic:
- Coronary artery disease
- Non-coronary (coronary vasospasm; tachycarrhytmias; hypotension; hypertensive
crisis; valvular disease)
● Valvular disease:
- Aortic stenosis
- Mitral stenosis or pulmonic stenosis rarely present with chest pain
● Nonischemic:
- Pericarditis
- Myocarditis
- Acute aortic syndromes (acute aortic dissection; intramural aortic hematoma;
penetrating aortic ulcer)
Hyperadrenergic states:

● Stress-induced cardiomyopathy
● Cocaine intoxication
● Methamphetamine intoxication
● Pheochromocytoma

Gastrointestinal:

● GERD
● Oesophageal hypersensitivity to pain
● Abnormal motility pattern and achalasia
● Eosphageal rupture, perforation and foreign bodies
● Other causes of oesophagitis (medication induced[antibiotics, NSAIDS]; infections)
● Hiatus hernia

Pulmonary:

● Vaculature:
- Embolism
- Pulmonary hypertension or cor pulmonale
● Parenchyma:
- Pneumonia
- Cancer
- Sarcoidosis
● Asthma exacerbation and COPD
● Pleura and pleural space:
- Pneumothorax
- Pleuritis
- Pleural effusion
Mediastinal

Psychogenic:

● Panic disorder
● Depression
● Hypochondria
● Cardiac cancer or other phobias
