Safety Behavior Expectation


EXPECTATIONS FOR ALL PERSONNEL

 Be knowledgeable about and adhere to applicable


safety rules, policies, standards, and procedures,
which includes the proper use of P.P.E.
 Individually accept and be held accountable for
your own safety behavior & performance.
 Have zero incidents within your control.
 Correct safety hazards within your control, and
report safety hazards outside of your control.
 Intervene when an unsafe act is observed.
 Report all incidents/injuries as soon as possible.
 Proactively participate in the site safety process,
including the following:
 Setting/Tracking Personal Safety Goals & Plans for Trinseo,
 Participating on Safety Program, Investigation Teams
 Using Established Standard Operating Procedures when required

 Performing Pre-Task Hazard Analysis (PTA),


 Performing Safety On Purpose program (SOnP) for Trinseo,
 Completing Required Safety Training On-Time,
 Participating in Special Safety Awareness Activities,
 Etc.

Environmental, Safety and Health


Policy

At Trinseo, protecting people and the environment will be part of


everything we do and every decision we make. Each employee has a
responsibility in ensuring that our products and operations meet
applicable government or Trinseo standards, whichever is more stringent.
Our goal is to eliminate all injuries, prevent adverse environmental and
health impacts, reduce wastes and emissions and promote resource
conservation at every stage of the life cycle of our products. We will
report our progress and be responsive to the public.

Safe work behavior is a basic requirement of


every individual working within Trinseo
Merak Site. This responsibility is best
summarized by:

INTRODUCTION

This safety book contains the minimum safety guideline and procedures for performance
of work by all employees, suppliers, visitors, and contractors. Subsequent to publication,
should any rule or procedure set forth herein differ from applicable government or local
laws, the more stringent will apply. Trinseo Merak site has developed these rules and
procedures to enable us to attain an injury-free workplace.

These rules and procedures represent the foundation for safety in Trinseo Merak Site and
are an integral part of every job.

This Safety Guideline is effective September 2011, until superseded. An electronic


copy of the guideline can be found in the Trinseo computer network. The department
EH&S contact can guide anyone to the network file location.

Compliance with these rules and procedures is a condition of working at Trinseo


Merak Site.

DEFINITIONS

A. Must, Shall, Will - Indicate mandatory rules.

B. Should - Means highly recommended.

C. Can, May - Indicate suggestions or guidelines.

D. Production Leader - The Trinseo employee in line management with primary


responsibility for a specified plant, operating unit, or department.

E. Owner - A Trinseo employee associated with a Process/Functional Leader who has


been delegated responsibility by that Process/Functional Leader for the operations
of equipment and processes associated with a specified plant, operating unit, or
department.

F. Owner's Representative - An employee who has been delegated responsibility by a


Process/Functional Leader normally for the overseeing of a specific project or activity of
limited duration.

G. Owner's Designee - An employee who is extended the rights and privileges of an owner
by the owner for specified purposes. These designations shall be documented.
Employees acting, as backs up do not receive designee privileges unless extended and
documented by the owner.

H. Trinseo Supervision - An individual who has been given the authority to direct and
oversee the specific actions of a worker or work group.

I. Weapons – Guns, Ammunition, Bows, Cross-bows, Switchblade Knives, Explosives

J. Prompt Medical Attention – Immediately dial 6666 (Latex Plant) and 7777
(Polystyrene Plant) to request emergency guidance.

TRINSEOMERAK SITE SAFETY GUIDELINES


A. RESPONSIBILITY FOR SAFETY

1. Compliance with Standards and Rules

All individuals while on the Trinseo Merak Site must comply with Trinseo
safety standards, site safety rules, and facility safety policies or procedures in
which they are working.

2. Individual Actions

All personnel are expected to act with respect, responsibility and common sense.
Do not engage in horseplay.

3. Personal Clothing

a. Where protective work clothes are not provided, each employee is expected
to wear personal clothing that is safe and proper for the job. Individuals
other than those working in an office type environment shall wear as a
minimum, a shirt with long sleeves, long trousers and closed-toed leather
or leather like shoes while on the job.

b. Loose clothing, jewelry or unconstrained long hair must not be worn around
machinery or moving equipment.

c. Individuals, other than those working in an office, shall not wear any type
of jewelry (including rings, necklaces, etc.) that is exposed. Small stud type
earrings worn in the lower part of the ear are allowed for those individuals
making observations or performing laboratory work. Another exception is
a wristwatch, as long as the individual is not working on or near
electrically energized equipment or welders wearing long cuff welding
gloves.

4. Contact Lenses

Contact lenses shall not be worn in plant process boundaries

5. Wearing of Rings

a. Rings must not be worn while in plant process areas and laboratories or in
other locations while engaging in activities related to process operation,
maintenance, construction, trucking operations or food service.5.....Gate
Pass requirement

Positive identification for entering and leaving a Site / facility gate is required for
personnel and goods (Vehicle, tools, product , raw material, etc)

6. ID Badge Requirement

ID badge shall be worn at all times within Site / Plant premises

7. Safety Orientation

Safety orientation / Induction is required for every one prior to work at Trinseo
premises. Safety induction will be given by a qualified facilitator

8. Visitor

Visitor must be escorted by a Trinseo representative. The host will ensure that
the visitor is safe and visitor’s activities are in compliance with Trinseo safety
requirement.

9. Weapons or Ammunition

Weapons or ammunition are not permitted on Trinseo property unless prior


approval is obtained from Facility Leader.

10. Voiding Safety Devices

Voiding Safety devices must follow MOC process. Contingency plan shall be
required before voiding safety devices

B. VEHICLES AND TRAFFIC SAFETY

1. Traffic Rules

a. Drivers must possess a valid driver's license.

b. Drivers must obey all traffic signs and rules in the plant.

c. Drivers must follow the instructions of emergency response personnel.

d. The maximum speed limit on the plant roads is 20 km/hr

e. Failure to follow traffic rules may constitute losing driving privileges.

f. Drivers while driving on Trinseo roadways will not use cell phones,
including hands free cell phones..

2. Seat Belts and Passenger

a. All occupants in passenger Trinseo or Trinseo lease vehicles shall wear seat
belts.

b. All occupants in the cab of pickups and other trucks shall wear seat belts.

c. No passenger allowed in the back of the truck.

d. Vehicles used to transport people shall have seats for the number of people to
be carried.

3. Motorcycles

a. Motorcycles shall not be ridden in plant and all motorcycles shall be parked
in Admin Parking area

4. Bicycles

a. bicycles shall not be ridden in Trinseo Merak Site, Except for Security Patrol
in the plant

5. Fork Trucks

a. Drivers of fork trucks should minimize travel on main plant roadways.


Drivers must travel on the side of the road.

b. Fork trucks shall not be driven on main plant roadways at night unless
equipped with headlights, a taillights and a flashing yellow light(s).

7. Vehicle Accidents

All motor vehicle accidents occurring on Trinseo property, as well as those


involving Trinseo employees on company business off-site, must be reported to
Facility Leader and EHS Delivery immediately.

8. Parking

Vehicles must not be parked in locations, which block plant roadways,


access roads, delivery spots, or fire lanes. Vehicles must be parked in designated
parking areas only, except vehicles used for maintenance or servicing activities
and documented on a Safe Work Permit or equivalent.

9. Unattended Vehicles

A motor vehicle engine must not be left running if the vehicle is unattended. This
does not include parking brakes on automobiles or pickups.

NOTE: "Unattended" means that the driver has left the normal control position
of the vehicle.

10. Passengers on Mobile Cranes, etc.

Within Trinseo secured property, passengers may ride on equipment such as


mobile cranes, winch trucks, or flat beds if adequate provision has been made for
seats.

11. Vehicle Capacity

The total load shall not exceed the capacity of the vehicle.

12. Repair of Vehicles

a. Doing repair on vehicles is prohibited. The only exceptions are changing a


tire or jump starting a vehicle.
b. When jump starting a vehicle, monogoggles must be worn when attaching or
detaching jumper cables.
c. The negative side of the jumper cables must be placed on a ground other than
the negative post on the battery.

13. Towing Vehicles

a. A vehicle shall not be pushed by another vehicle. Vehicles can only be pulled
or carried by those designed for vehicle towing purposes.

b. In the event that a wrecker must be called, prior permission must be received
from Security before it will be allowed to enter the plant.

14. Dragging Materials

Dragging or skidding material or objects along plant roadways is prohibited.

15. Moving Mobile and Heavy Loads

a. Prior to moving extremely heavy loads on plant roads, the Trinseo


supervisor or owner's representative responsible for the move shall contact
Security for proper routing of the load.

b. Authorization shall be obtained from EHS Delivery for all cranes above the
50-ton capacity outside of block perimeters. Axle loads and spacing shall be
calculated for all cranes above the 50-ton classification prior to crossing any
bridge.

c. When moving fixed boom cranes on Trinseo roads, a flagman shall be


provided to clear traffic at intersections.

d. When moving retractable boom cranes on Trinseo roads, the boom shall be
fully retracted.

e. The following rules pertain to all mobile cranes shall follow crane work
check list :

f. Oversized and/or Heavy Load routing should be arranged through Security.


Security will designate the appropriate routing and an escort.

g. Cranes and Tractor-Trailers must abide by all traffic rules while in the plant.

C. PERSONAL PROTECTIVE CLOTHING AND EQUIPMENT

1. General Requirements

a. The Facility Leader must establish the required minimum protective clothing
and equipment for their area of responsibility in accordance with the
Personal Protective Equipment Standards.

b. Personnel must wear protective clothing and equipment in the correct manner
as required.

c. Unless required for work being done, protective clothing and equipment is
not mandatory while in an office, control room, lunch room, change room,
going to and from parking lots and buildings by designated routes, or
walking main roadways.

d. All personal protective equipment shall meet the appropriate PPE standard
and in good condition

e. Personal protective clothing and equipment shall not be misused or altered in


such a way that their original protective features are rendered less effective.

2. Foot Protection

a. All protective footwear shall comply with ASTM F2413 (formerly ANSI
Z41).
b. Canvas, cloth, high heels (>1”), open-toed or open-back shoes must not be
worn in any non-office environment. Office workers must have shoes
available to wear that are suitable for an emergency evacuation, crossing
parking lots, uneven surfaces and climbing stairs if necessary. No bare
feet / flip-flops are allowed in Trinseo Merak Site other than in a
change/shower room.

3. Eye Protection

a. Approved safety glasses must be worn as minimum eye protection when in


non-office environments. Safety glasses must also be worn in offices or
buildings while doing maintenance or construction tasksA minimum of
ANSI Z87.1 safety glasses with side shields are required in shops, labs,
warehouses, process areas unless otherwise designated; as well as in
offices or buildings while doing maintenance or construction tasks.
Removable side-shield attachments shall meet ANSI Z87.1 requirements.

b. Areas/task requiring chemical goggles must be designated. The headband on


chemical goggles must be worn around one’s head, not around the hard hat.
One should also look down while removing goggles.

c. Safety glasses, chemical goggles with dark tint lenses should not be worn at
night or in indoor locations during the day unless wearing protective eyewear
for gas or electrical cutting or welding, or unless there is a prescribed medical
requirement for the dark tint lenses.

d. A face shield, in addition to either side-shield safety glasses or chemical


goggles, is required according to PPE grid.

e. Side-shield safety glasses are required under a welding hood while arc
welding.

f. Welding goggles shall be used for all gas welding or cutting, and welding
hoods shall be used for all electric welding and cutting.

4. Head Protection

a. All protective helmets shall comply with ANSI Z89.1.


b. Hard hats with Chin strap must be worn in plant process areas. Facility
Leader will specify the required head protection in all other locations.

c. Bump caps and metal hard hats are not permitted.

d. Hard hats should be regularly inspected and replaced according to the


manufacturer's recommendation.

e. Safety hard hats whose structural integrity has been altered by cracks, holes
or contact with organic solvents shall not be used. Intentional painting of
hard hats is not permitted.

f. Wearing of a welding hood does not preclude the use of a hard hat. The
wearing of a hard hat under a welding hood is dependent on the nature of the
job, and the Facility Leader will define its requirement.

5. Respiratory Protection

All respiratory program requirements in Trinseo Merak Site will be


accomplished in accordance with the Global Respiratory Protection Standard.

Personnel required by their job to wear nose-mouth respirators, full-face gas


masks, fresh-air masks, Self-Contained Breathing Apparatus (SCBA) or other
such protective equipment shall be clean shaven where the mask seal surface
contacts the face. These personnel shall be trained, fit tested and medically
approved to wear such equipment.

6. Hearing Protection

All hearing conservation program requirements in Trinseo Merak Site will be


accomplished in accordance with the Global Hearing Conservation Standards.
Hearing protection shall be worn when required

a. Approved gloves shall be worn for all operations, maintenance or


construction works unless they pose a greater hazard when worn for a
particular job.

b. The types of gloves used are to be established by the site / plant and must be
specified in PPE Grid, in specific written job procedures or during the work
permitting process.

c. Job procedures or work permitting process may specify the exceptions for
those activities when wearing gloves may pose a greater hazard to the
individual. Examples include, but are not limited to:
 working with high torque rotating equipment (this may not include
some hand-held power tools)
 heavy equipment operation (e.g., forktrucks, cranes, etc.)
 working with small parts requiring high dexterity

8. Water Hazard Protection

a. When working over or near water and the danger of drowning exists, an
approved life jacket must be worn or fall protection must be used.

b. When working over open water, ring buoys with at least 25 meter of line
shall be readily available for emergency use. Distance between ring buoys
shall not exceed 60 meter. Also, at least one lifesaving skiff shall be
immediately available.

9. Fall Protection

Fall protection must be used for elevated work. Fall protection must be used all
the time when working on elevation (high of 1.8 m or greater) and follow Fall
prevention procedure in elevated work standard

D. TREATMENT OF INJURIES OR ILLNESSES

1. Get Prompt Treatment

a. All injuries or illnesses must receive prompt approved medical attention.

b. All injuries or illnesses must be promptly reported to the injured employee’s


supervisor and to the Trinseo supervision of the area in which the incident
occurred.

2. Late Developing Injury or Illness

a. Any late developing (or aggravated) injury or illness must receive prompt
approved medical attention as soon as the conditions or symptoms are
recognized.

b. Late developing injuries or illnesses must be reported to the Trinseo


supervision of the area or block in which the incident occurred.

3. Personal Medical Treatments

a. Personal injuries, illnesses or medical treatments that may be aggravated by


the type of work performed on the job must be reported to the employee’s
supervisor.

b. Depending on the nature of a personal injury, illness or medical treatment,


the employee’s supervisor may require an employee to review his/her case
with Occupational Health before returning to work.

E. MISCELLANEOUS WORK REQUIREMENTS

1. Piping Specifications

Maintenance and construction shall be done in accordance with the appropriate


technology piping and equipment specifications. Deviations from written
specifications require compliance with Management of Change procedure.

2. Isolation of Energy Sources

a. The complete requirements for Isolation of Energy Sources are in the


Global Isolation of Energy Sources Standard.

b. All personnel must receive annual training in the Isolation of Energy


Sources safety standard.

c. Violations of Isolation of Energy Sources Safety Standard shall result in


disciplinary action.

3. Safe Work Permits

a. The complete requirements for Safe Work Permits are in the Global Safe
Work Permitting Standard.

b. A Safe Work Permit is mandatory for:

 Hot Work (where flammable or combustible material may be present),


 Confined Space Entry,

 Excavation work,
 Radiography,
 Critical Lifts,
 Lifts of personnel on suspended work platforms which are held or
supported by cranes,
 jobs required by department policy, or
 jobs when requested by the personnel doing the work.

4. Safety Shower/Eye Bath Location

Personnel must determine where the nearest Safety Shower and Eye Bath
facilities are located before commencing work. Testing of the nearest safety
shower or eye bath should occur before commencing work.

5. Cleaning with Compressed Air

Compressed air shall not be used to clean clothing or skin. If air is used for
cleaning work areas, etc., the pressure must be controlled at 30 psi or less.

6. Dropping/Throwing Tools

Tools, materials, or equipment shall not be dropped or thrown from one level to
another unless precautions have been taken against possible injury to personnel
or damage to equipment.

7. Barricades

Barricades must be erected to prevent or limit access to an area where a


temporary hazard exists or to warn personnel of a temporary hazard in an area.

8. Walking on Pipe

a. Walking on elevated pipelines requires fall protection.


b. Walking on pipelines at grade without fall protection must have written
approval of the Production Leader.

9. Walking on Cable Trays

Walking, crawling, or stepping on cable trays is prohibited unless the worker


follows a written cable tray entry procedure which has been approved by the
owner's Production Leader.

10. Walking on Corrugated Roofing

a. Walking on or otherwise putting one's body weight on corrugated roofing


without making provision for supporting that weight is prohibited.

b. Warning signs must be posted if corrugated roofing skylights are present on a


building. Warning signs must be posted at the access point to the roof and
posted or stenciled around skylights.

11. Venting or Dumping

Flammable materials or chemicals for cleaning – including, but not limited to


solvents, gasoline, oil, and hydraulic fluid – used by maintenance, construction
or service groups shall not be vented or dumped without the authority of the
appropriate EH&S Coordinator or Production Leader.

12. Working Above Grade

Climbing or working on equipment above grade but below 1.8 m without a


platform, ladder or other device designed for human occupancy is prohibited.

F. FIRE PROTECTION SYSTEMS

1. General Requirement

Materials or equipment must not be stacked or stored in locations that prevent


access to fire protection systems or their controls.

2. Fire or Smoke Detection Systems

Fire or smoke detection systems shall not be disabled without prior notification
of the Production Leader.

3. Fire Suppression Systems

a. Fire suppression systems shall not be disabled without prior notification of


the fire department.

b. Fire suppression systems (e.g., deluge systems, halon systems, etc.) that are
taken out of service must not be tagged or locked with a lockout/tagout
system.

c. Any time a fire suppression system is taken out of service or disabled, an


alternate fire protection plan must be developed.

G. MACHINERY AND TOOLS

1. General Requirement

Tools shall be used as their design was intended. Modification of an existing


tool, replication of an existing tool or creation of a new tool must be reviewed
and approved by the Production Leader and Maintenance Leader.

2. Defective Tools and Equipment

Defective tools and equipment shall be removed from service for repair or
disposed of immediately. Tools and equipment that are to be repaired must have
a tag attached stating the nature of the defect(s).

3. Moving Machinery

Machinery with moving parts or process equipment in service must be shut down
and isolated from the power source before making adjustments or repairs. If shut
down is not feasible, a written procedure specific for the job must be followed.

4. Guards on Moving Parts

Machinery, tools (including portable grinders and buffers), and equipment with
exposed gears, belts, power transmission couplings, etc., shall not be operated
without effective guards, unless approval for specific jobs is obtained from the
department head or owner’s representative.

5. Refuel of Equipment

a. Vehicles or equipment with gasoline engines shall not be refueled with its
engine running or while it is near any predictable source of ignition, such as
welding operations.

b. Fuel tanks on equipment powered by diesel engines shall not be filled with
the engine running if during the process it is possible to spill fuel on a hot
engine or manifold.

c. Portable fuel containers must not be filled in the bed of pickup trucks or
inside vehicles.

d. Locking fuel nozzles will be removed or permanently disabled from all


fueling devices/equipment. All fuel dispensing will be done with 100% hand

control on the filling device. No trigger props or other methods to hold a


fuel-dispensing handle open shall be used.

6. Knives

a. Safety Knives must use and follow the Cut Prevention/Knife Policy.

b. Kevlar cut resistant gloves must be worn when using knives on the job.
.

7. Portable Powered Tools

a. Manually held pneumatic, electric powered or hydraulic powered tools shall


not be locked or held mechanically in the "on" position.

b. All portable electric tools or equipment (including extension cords) shall be


inspected prior to use for potential shock hazards and according to their
required inspection schedules. Portable tools or equipment that are past due
on their scheduled inspection shall not be used until the inspection
requirements are met.

c. All portable electric tools or equipment shall be isolated from the power
source before making adjustments or changing drill bit, blade, etc.

8. Pneumatic Tools

a. Excess flow valves must be placed at the air source when using hoses greater
than 1/2" I.D. on portable tools.

b. Hoses must be depressurized prior to changing pneumatic equipment unless


the fittings are designed to be disconnected under pressure.

c. All pneumatic hand tools will have a block and bleed or quick connect
within 3 meter of the tool and at the header.

9. Grinders and Buffers

a. Tool rests on grinders shall be set not more than one-eighth inch from the
wheel and level with the center line of the wheel. Tongue guards on grinders
shall be set not more than one-quarter inch from the wheel.

b. Buffers are not to be operated with tool rests. Guards on buffers shall be cut
back at the bottom approximately 30 degrees behind the vertical center line
of the guard.

c. Hand held grinders must be used with Dead Man switch.

10. Wrenches on Rotating Equipment

Wrenches must not be used to rotate a pump shaft or similar rotating equipment
unless covered by a specific written procedure.

11. Quick Connectors

a. Quick connectors used for air and cold water (under 140F) must not be
compatible with quick connectors used for nitrogen or other services.

b. To ensure positive locking at all times, hose connectors shall be in good


condition and not abused. Abrasive blast hose connections shall be the
safety lock type, which prevent accidental disengagement.

12. Cheaters

Cheater devices, such as a pipe used as an extended handle on a tool, must only
be used with written approval of the Production Leader.

13. Quick Opening Valves

Quick opening valves – such as, quarter-turn ball valves, butterfly valves, etc. –
on the end of process or product lines containing hazardous materials must be
plugged or blinded when not in use.

14. Powder Actuated Tools

Powder actuated tools must not be used without written approval of the
Production Leader.

15. Welding Leads

a. To minimize the possibility of intermediate arcing during arc welding, the


grounding lead must be connected as close as practical to the work piece and
not have a splice within 3 meter of the stinger end.

b. When arc welding is to be done in an area restricted by a Safe Work Permit,


the grounding lead shall be connected to the work piece within the limits
specified in the permit.

c. All leads and lead connections shall be in good condition and free of any
exposed conductors, and must be inspected prior to use.

16. Wet Cell Batteries

Chemical goggles must be worn when installing jumpers or handling wet cell
batteries, including the "maintenance-free" types.

17. Jack Hammer Operations

A face shield, double hearing protection, toe and metatarsal protectors must be
worn when using a hand-held jack hammer for breaking concrete. Personnel in
close proximity of jack hammer operations shall also wear a face shield, double
hearing protection, toe and metatarsal protectors.

18. Soil Compacting Equipment

Toe and metatarsal protectors must be worn when using hand-held soil
compacting equipment such as a dirt tamper or packer.

19. Lifting Tools and Equipment

Hand tools and parts may only be lifted by devices designed for this purpose
(example: canvas-lifting bags, etc.). Buckets are not to be used for lifting tools
and parts with any means other than a person’s hand.

H. CHEMICALS AND MATERIALS

1. Chemical Hazard Identification

a. Storage tanks with 5000 gallons or greater in capacity containing materials


that may create a hazard in a fire must be identified with NFPA Fire Hazards
of Materials symbols.

b. Storage tanks located outside a process area must be labeled with the Owner
Department Name and 24 hour phone number or control center.

c. All non-Trinseo owned storage tanks except those on service vehicles must
be identified with NFPA Fire Hazards and Materials symbols and be labeled
with owner company name and 24 hour phone number.

d. All portable containers must be labeled with an appropriate hazard


identification GHS label, which includes the description of the contents, any
known hazards associated with the contents, user 24 hour phone number
and identification of Trinseo user/business. All agency and product
regulations will be met.

2. Glass Containers

Glass containers with one gallon or more in capacity shall be transported in a


shipping crate or other protective container or coating. Owners shall ensure
glass containers of any size shall not be subject to breakage in transit.

3. Potable Water Cross Connection

Potable water systems must not be connected to any other system.

4. Industrial Gases Used in Buildings or Confined Areas

a. Adequate provisions must be made to prevent an oxygen deficiency when


nitrogen or other industrial gases are used inside enclosures such as shops,
labs, wind breaks, vessels, pipes or welding enclosures.

b. Adequate provisions must be made to control personnel access to confined


areas such as analyzer houses which have nitrogen or other industrial gases
used inside.

5. Pressuring Drums or Portable Containers

a. Drums or portable containers shall not be pressured or padded in order to


remove contents.

b. While pumping into a drum or portable container, provision must be made to


vent vapor.

c. When a drum or portable container is heated, provision shall be made to


avoid over pressuring and rupturing the drum.

I. PORTABLE LADDERS

1. General Requirements

a. Ladders shall be non-conductive, industrial strength, and capacity rated for a


minimum of 150 kg. Tank ladders and scaffold ladders on scaffolds are
excluded. Ladders used in process areas shall be compatible with the
chemicals located in that area (consider chemical attack).

b. The design capacity shall not be exceeded (total weight of person plus
materials).

c. The user is responsible for visually inspecting a ladder before use and
ensuring that the ladder capacity is not exceeded. Any ladder found defective

shall be tagged with a “Warning” tag, removed from service, and repaired or
destroyed.

d. Wooden ladders are not allowed to be used within site.

e. Ladders are not to be used as pencil or scaffold boards.

f. Both hands must be used for climbing or three point of contact is used.

g. Only one person is permitted to climb a ladder at a time.

h. A ladder shall not be used to gain access to a roof unless the top of the ladder
exceeds at least 1.8 meter above the point of support at eaves, gutter, or
roofline.

2. Stepladders

a. Stepladders over 3 meter must be tied off at the top or held by another worker
if side supports are not provided for the ladder.

b. Personnel must not stand on the cap or the step below the cap.

c. Stepladders must not be used as straight ladders.

3. Straight and Extension Ladders

a. Top sections of extension ladders shall not be used by themselves except for
restricted entry into vessels, furnaces, and for scaffold access when properly
tied off.

b. Straight and extension ladders must be tied off within three rungs of the top.
Ladders must be held by another person while being tied off.

c. Extension ladders shall have one of the lower rungs of the upper section tied
to the adjacent rung of the lower section when in use.

d. Straight and extension ladders must be equipped with nonskid feet.

e. Straight and extension ladders shall be placed at such an angle that the base
is one-fourth of the working length of the ladder out from the top support
(e.g., 6 meter up, 1.5 meter out, or 1 rung out for each 4 rungs up).

f. The maximum length for straight ladders is 7.5 meter, and the maximum
length for extension ladders is 7.5 meter.

J. PLATFORMS AND SCAFFOLDS

1. Platforms

a. Make shift platforms, such as stacked material, chairs, boxes, or drums shall
not be used.

b. Rolling platforms shall be utilized according to the manufacturer's


recommendations, not altered in any way, and not ridden while being moved.

2. Scaffolds

a. Users of scaffolds must be trained in scaffold assess by EH&S Team.

b. Only trained competent personnel shall erect, move, dismantle or modify


scaffolds.

c. Scaffolds must be inspected prior to use.

d. All work levels of a scaffold, if practical, must be fully planked. Fall


protection is required if working from an incomplete scaffold.

e. An access ladder or equivalent must be used to access or egress work levels


of scaffolds.

f. Scaffolds must not be used as material hoist towers or for mounting derricks
without first determining the loads and stress involved.

g. The maximum loading for a “standard” scaffold – i.e., erected from the
ground or a floor of a structure – is three people and a dead load not to
exceed 90 kg or a total distributed load not to exceed 360 kg. The maximum
loading for a suspended scaffold is two people and a dead load not to exceed
90 kg or a total distributed load not to exceed 270 kg.

h. All scaffold platforms and tubes shall be free of tools, trash, etc., before
calling in removal.

i. Scaffold tubes must not to be used to store tools (knives, wrenches, welding
rods, etc.).

j. Scaffolds shall be equipped with handrails, midrails, and toe boards except
on owner representative's written approval.

k. A tagging system will be used to identify hazards on complete and


incomplete scaffolds.

l. When toe boards are missing the area around the scaffold must be barricaded
with warning tags stating “Danger overhead – No toeboards on scaffold.

m. Rolling tower scaffolds must be free of personnel, material, and equipment


before being moved. Caster brakes on rolling tower scaffolds must be locked
while in use.

n. Scaffolds shall not be built within 3 meter of any energized uninsulated


electrical lines without proper insulating barriers.

K. ELECTRICAL

1. General Requirements for Electrical Work

a. Work on or adjacent to uninsulated electrically energized equipment must not


be done without the permission of the owner's representative and must be in
accordance with specific written procedures.

b. Written procedures must be followed when operating electrical equipment


(e.g., electrical switchgear, motor control centers, transformers, disconnects,
etc.) powered by 440 volts AC or 250 volts DC or above.

c. Written clearing procedures and permits are required before working on


electrical equipment (e.g., electrical switch gear, motor control centers,
transformers, disconnects, etc.) which has been previously energized.

d. An Assured Equipment Ground Conductor Program, Ground Fault Circuit


Interrupters (GFCI's), or double insulated tools shall be used to operate
portable electric tools or equipment (including extension cords).

e. Placement of underground electrical systems and ground rods require an


excavation permit.

f. Permanent electrical grounding connections or systems, which are disturbed


or damaged, must be reported immediately to the owner's representative.

g. Portable generators, greater than 50 volts but less than 700 volts, which
are not part of a permanent wiring system, must be inspected before use
and annually. For detail please refer to Electrical Work Practice guideline

Receptacles of 380volt plugs must not be plugged into or unplugged unless they
are de-energized by an open switch.

3. Working near Energized Power Lines

a. Authorization must be obtained from the Electrical Maintenance before


operating power equipment (such as mobile hoisting, personnel aerial lift,
drilling, or dirt moving machinery) within 3 meter of the edge of the
electrical distribution right-of-way.

b. Power equipment (such as mobile hoisting, personnel aerial lift, drilling, or


dirt moving machinery) shall not be operated within 3 meter of any energized
uninsulated electrical lines without proper insulating barriers.

c. Any work where personnel or equipment will be within the following


distances of any overhead power line requires approval from the owner. This
includes cranes, condor lifts, bucket trucks, scaffolds, ladders, and other
types of elevated work platforms.

 6 meter for power lines.


 15 meter from any power line if a crane suspended man basket is being
used.

L. RADIATION

1. The Radiation Safety Officer must be contacted prior to purchasing, installing,


moving, transferring, or disposing of radioactive devices and materials.

2. Industrial radiography work must conform to the procedures and training


requirements.

M. ASBESTOS

All asbestos work in Trinseo Merak Site will be accomplished in accordance with
the government regulation regarding Asbestos.

N. LEAD

All work in Trinseo Merak Site associated with lead or lead containing substances
will be accomplished in accordance with the government regulation.

O. NON-TRINSEO COMPANIES WORKING AT TRINSEO

Non-Trinseo Company that will be working at Trinseo Merak Site shall follow all
applicable safety rules related to their scope of work that written in Non Trinseo
Service standard.

Mobile phone usage for Non Trinseo and Non Routine Contractor will be minimized
on site.
- Mobile phone is prohibited to be used during driving or walking around the
site.
- Mobile phone is prohibited to carry out in Process area.
- Project contractor and transporter driver shall store a Mobile phone in
Security office.
- Visitors shall get permission from It’s project managers to carry out their
mobile phone beyond the security office.

P. VARIANCES TO STANDARDS AND RULES

The Responsible Care Leader must approve variances to Trinseo Merak Site Safety
Standards and Rules in writing. All variances must be issued for a specific length of
time and may be renewed. The Site is responsible for maintaining a current list of all
variances, which have been approved. (Refer to the Global Variance Standar
