AFFIDAVIT OF DISCREPANCY

I, MARGIE MATIN-AO PITALLAR, of legal age, Filipino citizen and a

resident of Pasay, San Roque, Zamboanga City, Philippines, after having been duly
sworn to in accordance with law, hereby depose and say that:

1. I was born on June 25, 1969 at Malangas, Zamboanga Del Sur with a
registered complete name, MARGIE MATIN-AO PITALLAR, per
Certificate of Live Birth duly registered with the Office of the Municipal
Civil Registrar of Municipality of Malangas, Zamboanga Del Sur;

2. However, due to inadvertence my middle name was erroneously recorded

as MATINAO as reflected on my Professional Identification Card duly
issued by the Professional Regulation Commission (PRC) as well as on
other records with said office, when in truth and in fact my true and
correct middle name is MATIN-AO;

3. I hereby declare under oath and in good faith in order to establish the fact
that my true and correct middle name is MATIN-AO and not
MATINAO;

4. I execute this Affidavit to attest to the truth of the foregoing facts based
on personal knowledge and authentic records in compliance with the
requirements of PRC for the correction of my middle name and for
whatever legal purpose it may serve.
