AFFIDAVIT OF NON-OPERATION

We, Carmela Diana Dimasuay-Ponce and Jann Paolo Ponce, both of

legal age, married with one another, Filipinos, and with a business address
at 541 Rizal Avenue Barangay Calo Bay, Laguna, and after having been
sworn to in accordance with law hereby depose and state:

a. That we are the proprietors of an Angel’s Burger Food Stall

located at 541 Rizal Avenue Barangay Calo Bay, Laguna.
b. That we started operation without any employees on June 1,
2013 and manned the store ourselves.
c. That on January 2, 2016 we hired Jacqueline Arcillas Punzalan
as an employee for the said business.
d. That Jacqueline Arcillas Punzalan is our one and only employee
in history and at the moment in time.

been no other employee that worked in the said business other
than myself and the spouses Ponce.
