AFFIDAVIT OF LOSS

I, CATALINA A. TRILLO , Filipino, of legal age, residing at Sambag, San

Vicente Liloan Cebu , after having been sworn in accordance with law hereby
depose and state:

1. That I owned and in my possession SSS ID card, PAG-

IBIG ID card, TIN NO. ID card issued by BIR, PEOPLES
ID card, P.O. wallet card, REWARD card and RUSTAN
card.

2. That the said ID cards mentioned above are in my

wallet.

3. That sometime in May 14,2019, while making a transaction

in the municipal hall of Liloan my wallet were misplaced and
lost;

4. That I took pains to look for said wallet , specially where

my IDs mention above were in that wallet, but to no
avail.

5. That my wallet as well as my SSS ID card, PAG-IBIG ID

card, TIN NO. ID card issued by BIR, PEOPLES ID card,
P.O. wallet card, REWARD card and RUSTAN card are
now beyond recovery.

That I am executing this affidavit to attest to the truth of

the foregoing and to whatever legal purpose it may served.

1. That I owned and in my possession SSS ID card, , TIN

NO. ID card issued by BIR, PEOPLES ID card,
DISABILITY ID Card.

2. That the said ID cards mentioned above are in my

wallet.

3. That sometime in May 14, 2019, together with my wife

Catalina, Trillo we went to the municipal hall of Liloan to
make some transaction.

4. while making a transaction in the municipal hall of Liloan

my wallet were misplaced and loss

5. That I took pains to look for said wallet , specially where

my IDs mention above were in that wallet, but to no
avail.

6. That my wallet as well as my SSS ID card, TIN NO. ID

card issued by BIR, PEOPLES ID card, DISABILITY ID
CARD are now beyond recovery.
