Foreword

It is our pleasure to present to you the Annual Report of the Department of


Health–SOCCSKSARGEN REGION the accomplishments for the calendar year
2018.

This will serve as a reference for future formulation, planning and


management, and may prove vital in decision-making both at the local and national
levels.

We acknowledge and appreciate the support and dedication of our frontline


health workers in the provinces, cities, municipalities and up to the barangays for
delivering quality health and health-related services to our clientele.

Moreover, we extend our heartfelt gratitude to all our partners and


stakeholders in the health service, especially the Local Government Units, Non-
Government Organizations, other Government Organizations and the Civil Society
who are always behind us in all our undertakings.

The realization of this Annual Report is dedicated to all those who are behind

REGIONAL PROFILE
CY–2018

Historical Background

On July 7, 1975 under Presidential Decree No. 742, Region XII was created otherwise
known as Central Mindanao. Under this decree, Central Mindanao was composed of five (5)
provinces and three (3) cities, namely: Cotabato, Sultan Kudarat, Lanao del Norte, Lanao del Sur
and Maguindanao and the cities of Cotabato, Iligan, and Marawi.

On August 1, 1989, Republic Act No. 6734 was signed into law, which provided for the
creation of the Autonomous Region in Muslim Mindanao (ARMM). The provinces of Lanao del Sur
and Maguindanao were included in the composition of ARMM.

On October 12, 1990, Executive Order No. 429 was signed into law, defining the new
composition of Central Mindanao with Koronadal City as its Regional Administrative Center.
However, a restraining order was issued, resulting to the non-execution of the said EO. Thus, the
composition of Region XII prior to the issuance of EO 429 (i.e. Cotabato Province, Sultan Kudarat,
Lanao del Norte, Cotabato City, Iligan City and Marawi City) remained on status quo. Cotabato City
remained as the Regional Administrative Center of Region XII. It has been likewise designated as
the temporary regional center of the ARMM.

Then on, February 23, 1995, Republic Act 7901 “An Act Creating Region XIII to be known
as the Caraga Administrative Region, and for other purposes” was enacted. The law provided for
the transfer of Sultan Kudarat Province which is under Region XII to Region XI following the
transfer of Surigao del Sur to Caraga Region (Region XIII). However, in 1998, RA 8744 was passed
transferring back to Region XII the province of Sultan Kudarat.

Pursuant to the provisions of the Final Peace Agreement signed between the Government
of the Republic of the Philippines and the Moro National Liberation Front (MNLF) on September 2,
1996. Executive Order No. 371 was issued by His Excellency Fidel V. Ramos on October 2, 1996.
EO 371 established a Special Zone of Peace and Development (SZOPAD) in Southern Philippines,
which encompassed five regions, including Region XII.

On August 14, 2001, pursuant to the provision of RA 9054, a plebiscite was conducted in
the SZOPAD to determine the areas that shall comprise the ARMM. The cities of Marawi in Region
XII and Isabela in Region IX joined the ARMM. As a result of the reconfiguration of the ARMM,
Pres. Gloria Macapagal-Arroyo issued Executive Order No. 36 dated September 19, 2001, which
calls for the realignment of the regions in Mindanao. The province of Lanao del Norte and Iligan
City of Region XII were transferred to Region X. The provinces of South Cotabato, Sarangani, and
the cities of Koronadal and General Santos of Region XI became part of the new Region XII.

Thus, per Executive Order No. 36 dated September 19, 2001, “Providing for the
Reorganization of the Administrative Regions in Mindanao and for other purposes”, Region XII now
is called “SOCCSKSARGEN” and is composed of 4 provinces and 5 cities. These provinces are:
North Cotabato, Sarangani, South Cotabato and Sultan Kudarat. The five cities are: Cotabato City,
Koronadal City, Gen. Santos City, Kidapawan City and Tacurong City. Only 2 cities considered
Chartered cities namely: Cotabato City and Gen. Santos City. Kidapawan City, Koronadal City and
Tacurong City considered component cities of North Cotabato, South Cotabato and Sultan Kudarat
provinces respectively.

Geographic Facts

REGION XII is situated in the central and southwestern part of Mindanao bounded on the
North by the provinces of Lanao del Sur and Bukidnon; on the northwest by Maguindanao; on the
southwest by Mindanao Sea; and on the east by Davao del Sur and Davao City. It has a total land
area of 20,028.90 sq. km. Its area is about 22% of the total land area of Mindanao. Among the four
provinces in the region, North Cotabato has the biggest land area at 6,606.00 sq. km, which is
about 33% of the region’s land area. The province of South Cotabato has the smallest area among
the provinces at 3,421.60 sq. km. (Table 1)

Among the five cities of the region, Gen. Santos City has the biggest land area at 536.10
sq. km., while Tacurong City has the smallest with about 164.80 sq. km. (Table 1)

Topographically, SOCCSKSARGEN varies from flat, fertile plains to irregular landscape to


wide valleys, scattered hills and intensive mountain ranges. About half of the total land area of the
region’s land resources fall within the 0-500 m in elevation. It also has an extensive coastline which
stretches to 320 kilometers. The mountain ranges of the region are located in the northern and
eastern portions of Cotabato, and in the central and southwestern portions spanning through the
three provinces of Sultan Kudarat, South Cotabato and Sarangani.

Demographic Profile

The total population of REGION XII in 2018 was 4,806,840 (Per Department Memorandum
No. 2017-0073, dated February 8, 2017. Based on Philippine Statistics Authority (PSA) 2015
census of population. North Cotabato Province (including Kidapawan City) has the largest
population among the provinces of the region with 1,472,829 or 30.64% of the total, followed by
South Cotabato (including Koronadal City) with 967,785 or 20.13% of the total, Sultan Kudarat
(including Tacurong City) with 850,552 or 17.69% of the total and Sarangani with 571,167 or
11.88% of the total. Among the chartered cities Gen. Santos City has the largest population with
628,540 or 13.08% of the total and Cotabato City with 315,967 or 6.57% of the total. (Table 2)

The Region has population density of 239 persons/sq. km. Among the cities, Cotabato City
has the highest population density at 1,795 persons/sq. km, while Kidapawan City has the lowest
with 449 persons/sq. km. For the provinces, South Cotabato has the highest population density with
228 persons/sq. km., while Sarangani has the lowest with 144 persons/sq. km. (Table 1).

REGION XII has an average annual growth rate of 2.28 percent, which is about 0.44
percentage point higher than the national growth of 1.84 percent (2000-2015 growth rate).

Among the provinces, North Cotabato has the fastest growing population, with an annual
average growth rate of 2.41 percent, while Cotabato City has the fastest growing population among
chartered cities with 4.03 percent average annual growth rate (PSA).

The total population of Region XII is almost evenly shared by both male and female with
50.95 percent and 49.05 percent respectively. Most of the population is on the 15-49 years old age
bracket, which comprises 50.61%. (Table 3/Figure 2)

The Regional Health Office No. XII – Cotabato City

In accordance with Presidential Decree 742, Administrative Order No. 278, s. 1976 which
was signed into law on February 12, 1976. It provided for the organization and activation of the
Regional Health Office XII.

Until 1989, the office's mandate was based on Executive Order 119, which provided the
Department of Health the sole responsibility in the formulation, implementation and coordination of
policies and programs in the field of health. With the advent of Devolution, the DOH function on
service delivery was devolved/transferred to the Local Government Units, Section 52, IRR of the
Local Government Code of 1991.

The Department of Health underwent reengineering of its structure and functions, based
on Executive Order No. 102, signed on May 24, 1999 by then President Joseph Estrada. It
provided for the redirection of the functions and operations of the Department of Health.

The first phase of reengineering started at the DOH-Central Office where workforce was
cut by 50%. Most of these staff were deployed and reassigned to various Regional Health Offices
and retained facilities in the country. The second phase of reengineering, (for various regional
health offices) was fully implemented on August, 2013 per Executive Order 366 or otherwise called
“Rationalization of Government Agencies”. The Regional Health Offices are now called "Centers
for Health Development" as reflected in the General Appropriation Act of 2001. Out of Two
Hundred Thirty One (231) plantilla positions with One Hundred One (101) warm bodies, Sixty Three
(63) personnel availed to retire and Fifteen (15) are co-terminus with the incumbent (CTI’s).
Presently, there are One Hundred Thirty (130) plantilla position.

On March 18, 2014, per Regional Memorandum No. 054 s. 2014, the Center for Health
Development-SOCCSKSARGEN has been changed to Department of Health-Regional Office No.
XII.

HEALTH RESOURCES

Facilities

There were 185 licensed hospital facilities in SOCCSKSARGEN (Region XII), about 48%
are privately owned while the remaining 52% are either DOH retained hospital or public hospitals
under the different LGUs. On the provincial level, 66 or about 36% private and public hospitals are
found in North Cotabato while only 13 or 7% are found in Sarangani Province. Among chartered
cities 11 or 6% private & public hospitals are found in General Santos City (Table 27).

Supporting the hospital facilities in the region are the 56 main health centers, 5 City Health
Offices and 1,148 barangay health stations. North Cotabato has the most number of barangay
health Stations with 489 or 42%. (Table 9A)

Health Human Resources

SOCCSKSARGEN (Region XII) is being served by 118 Doctors, 272 Nurses, 13


Nutritionists, 62 Medical Technologists, 50 Dentists, 810 Midwives, 1 Sanitary Engineers and 85
Supervising/Sanitary Inspectors. Supporting these health personnel are the 712 Nurse Deployment
Program (NDP), 347 Rural Health Midwife Placement Program (RHMPP), 4 Doctors to the Barrio
Program, 8 Dentist Deployment Program, 72 Medical Technologist, 49 Universal Health Care
Health Implementer and 51 Public Health Associates (PHA) and the 7,972 Active Barangay Health
Workers. (Tables 9A-9C)

Regional Office No. XII has 130 plantilla positions. Office of the Director = 4 (Regional
Director, Assistant Regional Director, Administrative Assistant III and Administrative Aide III);
Management Support Division=40; Local Health Support Division=43, Regulation Licensing and
Enforcement Division=17; and Provincial Health Teams=26. As of December, 2018 One Hundred
Thirty (130) plantilla positions were already filled up.

HEALTH STATUS

Crude Birth Rate (CBR)

Region XII registered a total of 83,945 live births for 2018. Crude Birth Rate (CBR) is
computed at 17.42 per 1,000 population. An increase of 0.04% rate points compared to 2017
report.

Among the provinces, South Cotabato has the highest Crude Birth Rate (CBR) at 20.13
per 1,000 population, while Sultan Kudarat has the lowest at 15.22 per 1,000 population. Among
the chartered cities, Cotabato City has the highest at 22.98, while General Santos City has the
lowest at 16.98 per 1,000 population. (Table 13/Figure 10A)

Approximately 95.12% of the total Livebirths has a weight of 2,500 grams and above,
3.55% belong to low birth weights (2,500 grams and less) and 1.33% accounts for the unknown.
Deliveries attended by skilled birth attendants (Doctors, Nurses & Midwives) in health facilities,
Trained Birth Attendants (TBA) and Others accounted for 92.05%, 6.50% and 1.45% respectively.

Crude Death Rate (CDR)

For the year 2018 Region XII has total deaths of 16,705 resulting to a rate of 3.48 per
1,000 population, which showed an increase of 0.21% rate points compared to 2017 report.
Among the total deaths, 60% were males and 40% were females. Male mortality rate was at 2.08
while that for females was 1.39 per 1,000 population.

Among the provinces, South Cotabato has the highest Crude Death Rate (CDR) at 3.56
per 1,000 population, while Sarangani has the lowest at 2.28 per 1,000 population. Among the
chartered cities, General Santos City has the highest at 5.48, while Cotabato City has the lowest at
3.31 per 1,000 population. (Table 13/Figure 10C).

Infant Mortality Rate (IMR)

Infant deaths accounted for 535 or 3.20% of the total deaths of the region, with a rate of
6.37 per 1,000 livebirths. An increase of 26% compared to 2017 report. Among the provinces,
South Cotabato has the highest infant mortality rate of 10.93 per 1,000 livebirths, while Sarangani
has the lowest with 1.98 per 1,000 livebirths. Among the chartered cities, Cotabato City has the
highest rate with 4.82 per 1,000 livebirths, while General Santos City has the lowest rate with 3.94
per 1,000 livebirths. (Table 13/Figure 10C)

Leading Causes of Infant Mortality

Sepsis/Septicemia (A41.9); was the leading cause of Infant Mortality. Followed by:
Pneumonia, unspecified (J18.9); Prematurity/Preterm Baby (R06.03); Fetal Death In Utero (O36.4);
Respiratory Distress Syndrome (RO6.03); Asphyxia (R09.01); Congenital Malformation/Anomalies
(Q89.7); Congenital Heart Disease (Q24.9); Meningitis (G03.9); and Diarrheal Disease (R19.7).

Maternal Mortality Rate (MMR)

Maternal deaths accounted for 51 or 0.30% of the total deaths in the region, with a rate of
60.75 per 100,000 livebirths. An increase of 3.11 rate points, compared to 2017 report. Among the
provinces, Sultan Kudarat has the highest maternal mortality ratio of 100.43 per 100,000 livebirths,
while North Cotabato has the lowest maternal mortality ratio of 44.45 per 100,000 livebirths.
Among the chartered cities, General Santos City has the highest rate of 56.22 per 100,000
livebirths, while Cotabato City has the lowest with 27.54 per 100,000 livebirths. (Table 13/Figure
10E).

Leading Causes of Maternal Mortality

Postpartum Hemorrhage (O72) was the leading cause of Maternal Mortality with a rate of
22.63 per 100,000 livebirths followed by Eclampsia/Pre-eclampsia (O15.2) with a rate of 16.68 per
100,000 livebirths. The others were Uterine Atony (O62.2); Retained Placenta (O85); Puerperal
Sepsis (O48.0); and Placenta Abruption (O44.10). (Table 7/Figure 6)

Under Five Mortality Rate

Under Five Mortality accounted for 776 or 4.64% of the total deaths in the region, with a
rate of 9.24 per 1,000 livebirths. A decrease of 20.08% or 3.02 rate points, compared to 2017
report. Among the provinces, Sultan Kudarat has the highest under five mortality rate of 12.59 per
1,000 livebirths, while Sarangani has the lowest at 3.70 per 1,000 livebirths. Among the chartered
cities, Cotabato City has the highest, with the rate of 5.78 per 1,000 livebirths, while Gen. Santos
City has the lowest rate of 5.62 per 1,000 livebirths. (Table 13/Figure 10D)
