By signing this form, I freely and voluntarily give my consent to Sta.

Clara International Corporation,

its authorized representative and employees as well as its parent company, subsidiaries, affiliates,
partners, and service providers, if any, to collect, store, process, retain, access, share, and/or dispose
my personal data, whether manually or electronically, for purposes of this form or incidental thereto
and whatever legal purpose the Company may deem appropriate and necessary for as long as it
remains relevant or within the period allowed under the applicable law and regulations. I recognize
that the processing of my personal data is necessary for such purposes. I am aware of my right to be
informed, to access, to object, to erasure or blocking, to damages, to file a complaint, to rectify, and
to data portability; and I understand that there are procedures, conditions and certain exceptions in
the exercise such rights.

The foregoing personal information is freely and voluntarily submitted to the Bureau of Customs (BoC)
for purposes of application by Sta. Clara International Corporation as an Accredited Importer with the
reasonable belief that the BoC shall use contractual or other reasonable means to make certain that
proper safeguards are in place, to ensure the confidentiality, integrity and availability of any and all
the personal data processed, prevent its use for unauthorized purposes, and generally, comply with
the requirements of the Data Privacy Act of 2012, its Implementing Rules and Regulations, and
issuances of the National Privacy Commission in upholding the rights of the data subject whose
information is hereto provided, including the right to be informed, to access, to object, to erasure or
blocking, to damages, to file a complaint, to rectify, and to data portability.
