I, ELMER RESURRECCION CAMARSE, Petitioner in the above-

captioned case, of legal age, Filipino, after having been sworn, in accordance
with law, hereby depose and state that:

1. I am the Petitioner in the above-captioned case;

2. I have read and understood the contents hereof of this Petition for
Review, and the facts herein alleged are true and correct of my own
knowledge and based on available verifiable records;

3. I further certify that (a) I have not commenced any other action or
filed any claim involving the same issues in any court, tribunal or
quasi-judicial agency; (b) that to the best of my knowledge, no such
action or proceeding is pending in any court, tribunal or quasi-
judicial agency except the present petition; (c) and that should I
hereafter learn that such similar action or claim has been filed or is
pending before any other court, tribunal or agency, I undertake to
