DEED OF DONATION INTER VIVOS AND ACCEPTANCE

KNOW ALLMEN BY THESE PRESENTS:

This DEED OF DONATION AND ACCEPTANCE, made and executed this

________ day of ___________2 019, at______________________________,by
and between:

I, _____________________, of legal age, a citizen of the Republic of the

Philippines with permanent address at Rolling Hills Subd., Bgy. Kaligayahan,
Quezon City, hereinafter referred to as the “DONOR”.

________________________, of legal age, single, eldest child of the donor

with permanent address at Rolling Hills Subd., Bgy. Kaligayahan, Quezon City,
hereinafter referred to act as “DONEE”.

WITNESSETH: That-

WHEREAS, the DONOR is the absolute registered owner of a parcel of land

covered and embraced by TCT No. T__________ Lot 20, Block 35-B, BSD-04-
000950 of Lot 1-A Psd-176, of the Registry of Deeds for Tagbilaran City, Bohol
containing an area of Seventy Five (75) square meters, more or less, and particularly
described in the attached Technical Descriptions marked as Annex “A” and made
as an integral part of this Deed.

WHEREAS, the primary concern of the DONEE is to put, operate and maintain
the business of the donor in the said donated property;

NOW, THEREFORE, for and in consideration of the foregoing premises, the

DONOR by these presents does hereby cede, transfer and convey by way of
donation in favor of the DONEE, aforesaid parcel of land designated as Block-35,
Lot-20 of Bsd-04-00950 of Lot 1-A Psd-176 particularly described in the attached
Lot Descriptions mark as Annexes “A” situated in Tagbilaran City, Bohol containing
an aggregate area of 75 square meters subject to the following conditions:

1. The properties shall be used exclusively for Business purposes and may not
be used for other purposes;

2. The DONEE shall not alter or modify the use of properties nor cede, sell,
transfer, lease or any manner encumber or dispose of in whole or in part
without the written consent of the DONOR;

3. All expenses to be incurred in the registration of the Deed of Donation and

the transfer of titles with the Registry of Deed shall be for the account of the
DONEE;

4. The DONEE shall be responsible for the proper maintenance and upkeep of
the donated facilities;

5. The lots shall revert to the DONOR in the event that any of the above
conditions is violated or not complied with.

The foregoing conditions shall be annotated at the back of the titles to be issued
to the DONEE.

That the DONOR has reserved sufficient properties in its full possession and
enjoyment in accordance with the provisions of its charter.

The DONEE by these presents, does hereby accept with gratitude the herein
donation in accordance with the purpose above-stated.

IN WITNESS WHEREOF, the parties have hereunto signed this Deed on the
date and place first above written.

WE, the undersigned witnesses hereby attest and certify: That the foregoing
DONATION INTER VIVOS was executed by and consist of _____________ (_____)
pages, including this page where the Attestation of Witness and the
Acknowledgement before the Notary Public appears; That this was signed by the
DONOR and the DONEE in our presence, and we at their request, have signed
these presents on the last page hereof, and on the left-hand margin of each and
every page, in their presence, and in front of each and everyone of us.
