JUDICIAL AFFIDAVIT OF
MR. ALLISTAIR SHADOW MOON

I, ALLISTAIR SHADOW MOON, 32 years old, single, residing at

Barangay Poblacion, Zarraga, Iloilo, after having been sworn to in accordance with
the law do hereby depose and state:

That Atty. Nestor Jeremy B. Moreno is the counsel who conducted and
supervised my examination as an accused at MORENO and JALIPA LAW OFFICE,
123 Ana Rose Building, Iznart St., Iloilo City;

That I am answering the questions herein fully conscious that I do so under

oath and that I may be criminally liable for false testimony or perjury;
 That the following are the questions propounded by Atty. NESTOR JEREMY
MORENO and my answers are in ENGLISH LANGUAGE:

Q1: Do you swear to tell the truth and nothing but the truth?

A1: Yes Sir.

Q2: Can you state your personal circumstances?

A2: Yes sir. I am Allistair Shadow Moon, 32 years old, single, residing at Barangay
Poblacion, Zarraga, Iloilo.

Q: What do you do for a living?

A: I am an employee of LGU Zarraga, Iloilo.

Q3: What is your position or designation in the Local Government of the

Municipality of Zarraga, Iloilo?

A3: I am a Collecting Officer Sir.

Q4: Why are you here?

A4: To give my testimony freely and voluntarily and the same will constitute my
direct testimony in the above mentioned case.

Q5: Do you undertake to answer the questions to be asked of you, fully conscious
that you will do so under oath, and that you may face criminal liability for false
testimony or perjury?

A5: Yes sir.

Q: Where are you on January 20, 2017 at around _____?

A: I am at Zarragpublic market Sir.

Q: What are you doing at Zarraga public market?

A: I am doing my duty sir as a collecting officer.

Q: What is your duty as a collecting Officer?

A: During market day, I collect fees from the seller who are selling at the public
market Sir.
Q: Were you at Zarraga Public Market the whole day?
A: No Sir.

Q: At what time did you leave Zarraga public market?

A: At around ____ Sir.

Q: Where did you go from Zarraga Public Market?

A: I went to the Office Sir.

Q: Where is your office?

A: At the Municipal Treasurer’s Office Sir.

Q: What time did you arrived at the Municipal Treasurer’s Office?

A: I arrived at around _ Sir.

Q: When you arrived at the MTO, what did you do?

A: I went to my designated table Sir.

Q: What did you do on your table?

A: I counted the money I collected from the public.

Q: After that, what happened?

A: I happened to glance at the Municipal Treasurer’s Area and saw the COA
Auditors.

Q: What did you do when you saw the COA Auditors?

A: I asked my co-Collecting Officer if what do the COA Auditors doing inside the
Treasurer’s Office.

Q: What answer did you get?

A: I was told that the COA Auditors were conducting a cash examination.

Q: After you learned that the COA Auditors were conducting a cash examination,
what did you do?
A: I prepared my collections and accountable forms to be presented Sir.

Q: Why did you prepare your collections and accountable forms?

A: Because I was expecting the COA Auditors to simultaneously conduct a cash
examination of my cash and accounts as they usually do Sir.
Q: After you prepared your collections and accountable forms what happened next?
A: I stayed at my designated table and waited for the COA Auditors to present to me
a Cash Production Notice.

Q: Did the COA Auditors present to you a Cash Production Notice?

A: No Sir.

Q13: Do you have anything else to say?

A13: No more sir.
