Re: Sandra Peterson, D.O.B.:1/01/1923

This is to refer Mrs Peterson, a 96-year-old woman, into your care for further assistance and support
due to an upper respiratory infection. She is set to be discharged today.

On 20/03/2014, Mrs Peterson presented at the hospital with shortness of breath, dehydration and
bi-basal crackles sounds in her chest after she felt three times. At that time, tests were taken, but
none of them indicated any abnormalities; however, her blood pressure remains high even if an anti-
hypertensive was given. Moreover, she takes a large number of medications, which also includes
OTC tablets.

Mrs Peterson can ambulate independently with the help of a walker, but only for short distances,
and for long distances she needs assistance using a wheelchair.

In the view of the above, it would be greatly appreciated if you could provide Mrs Peterson home
visits because she needs help with her personal care and management with her daily medication.
Note that, she needs to continue her 500 mg Augmentin treatment twice per day for the next five
days, it is indicated to be taken at the start of each meal, 25 mg of Metropol two tablets daily as well
as 16 mg of Candesartan in the morning. Lastly, her daughter, Ann Macarthur, requested that her
mother needed to be educated about the danger of polypharmacy. If you require any further
