MEMORANDUM
2. ICOW the above reference, please be informed that the undersigned failed to
attend the 6:30 AM DPAR of PS-5 Personnel on Sept. 2-8, and Sept. 9-15, 2019 for the
reason that I was send to school my 5 years old daughter because her class start is 6:00
am to 11:00 am and on Sept. 5 and 12, 2019 I had a fallow-up check-up for my pneumonia
medication, failed to come early due to un-expected situation I failed to attend the said
DPAR.
3. That, the undersigned have no intention to defy legal order of the PNP
organization.
