PAY SCHEDULE.
Employees are paid on a bi-weekly basis every other Friday, one week in arrears. Each work week
begins on a Sunday and ends on a Saturday (e.g. a two-week pay period beginning on Sunday the
1st and ending on Saturday the 14th is paid on Friday the 21st).

Note: Sales employees are not paid bi-weekly; they are paid semi-monthly on the 15th and the last
day of the month.
Note: Your first check will always be a paper check, even if you signed up for direct deposit. All
direct deposits must go through a pre-note set-up, which verifies account information with the
bank.

BENEFITS ELIGIBILITY.
• All new hire’s benefits will be effective the first of the month following 60 days of
employment at Equinox. New hires have 30 days from their DOH to complete enrollment. If
enrollment is missed, they will not be eligible for Equinox benefits until Open Enrollment
unless they experience a Qualified Life Event (QLE). See “How to Enroll for Benefits” one-
sheeter.

TIME OFF POLICY.

• Vacation: Certain full-time employees are entitled to paid vacation time which is based on
your anniversary year, not the calendar year. Contact your Manager for any questions.
• Holidays: All salaried employees are entitled to 5 paid holidays each year: Memorial Day,
Independence Day, Labor Day, Thanksgiving, and Christmas. If a salaried employee is
required to work on any of these holidays, they will be entitled to one (1) alternate day off at
a mutually agreed upon date with their manager. Hourly employees are paid time and one-
half of their regular rate of pay for working New Year’s Day, Easter, Thanksgiving, and
Christmas. Holiday pay is not counted for the purpose of calculating an employee’s overtime
hours of work or overtime premiums unless required by applicable law. Contact your
Manager for any questions.
• Personal and Sick Time: Certain full-time salaried employees are entitled to 3 paid personal
days and 3 paid sick days after completing 3 months of employment. After that they’re
entitled to 3 paid personal and 3 paid sick days each calendar year. Those hired mid-year are
entitled to personal and sick time in their first year of employment according to the following
schedule:

Note: Some sick time polices vary by state. Unused vacation/personal/sick time can’t be carried
over to the following year and isn’t paid upon termination of employment except where required by
