Note : This sheet is for our reference only, so you need not give me very accurate answers
or trouble the referee for details right now, this is just to give me a general idea of the kind
of credibility he will enjoy as a referee.

Important Pointers: Please read each question and try to answer to the best of your ability.
The quality of the drafted Recommendation letter depends on the quality of your answers.
So, please answer each question in detail. You will need to send us a separate
questionnaire for all your referees.

You need not take your professor’s signature or college’s stamp on this document.

Instructions:
a. There are no right or wrong answers these are just to customize your
recommendation and get an idea at what level you have interacted and what kind
of relationship you share with him.
b. Please draw cross references with your resume if necessary, that is, which
activity to be associated with which specific referee

Questions:
1. Please define how long and in what context the referee has known you.
 2. What do you feel the person’s perception about you is (use adjectives or
phrases e.g. intelligent, dedicated etc)
*(Please elaborate on this answer and do not use only the adjectives that we have
already used as an example. )

3. Any specific achievement that your referee has been associated with.
(Research work, seminars, presentation, etc.)

4. Please provide an example where the referee feels that you have
participated in activities other than academics (Leadership positions,
extracurricular activities, and any other event or activity you have
interacted with the referee at a personal level directly or indirectly)

5. Technical skills he may feel you posses (eg design, process analysis or
programming)

6. MOST IMPORTANT OF ALL HOW AMENABLE IS HE LIKELY TO BE WHILE

Not very good may not be inclined to say very good things about me.
